We’re excited to welcome a new Family Physician to our clinic.
-Dr. Anthonia Imoudu

To support patients in our community who do not currently have a primary care provider, patient applications will be available for individuals who are not already under the care of another physician or nurse practitioner.

Applications will be available to local residents of Beaverlodge/Hythe and close surrounding area for pick up in person only. Numbers are limited. Please watch the page as applications tend to go fast.
1030 1Ave Beaverlodge.
One application per patient in your family is required (child, spouse etc). Application pick up is starting tomorrow April 9 at 1:30 pm.

Once your application is complete, please drop it back off to our office and our office will then call you for a first visit as appointments become available.

The Grande Prairie PCN has launched a new clinic called Preventative Care Clinic, which is for individuals who don’t have a primary care provider, but need preventative screenings done. They can assess eligibility and provide screenings for high blood pressure, high cholesterol, diabetes, breast cancer, colon cancer, cervical cancer, and lung cancer.

Have you noticed an extra black rolling garbage bin in your yard that isn’t supposed to be there? Our clinic is missing ours, possibly from the wind storm last week. The last four numbers are 0168. If you have it, or have seen it could you let our office know. 780-354-3044
