If you still require your medical records we have extended the pickup timeframe. Please call the clinic first for cost of your chart between 10 and 3. Wednesday the 21st will be the final day they can be picked up at clinic. Please share this post!

If you require copies of your medical records please call the clinic Monday to Friday from 10 AM to 3 PM and the steps involved will be explained. There will be a cost…cash only as we don’t have a debit machine.

NOTE: June 16th will be the last day you will have access to receive your medical records by pickup at the clinic.
