This week we joined the incredible nursing team at Brampton Civic Hospital for a Lunch & Learn on something that doesn’t always get spotlighted:
✨ the safe transition from hospital to home.

Those first 7–14 days post-discharge are when seniors are most vulnerable — mobility changes, medication routines, meal prep, hydration, and family overwhelm. That’s where our Hospital-to-Home Program steps in.

How we support after discharge:
✔ mobility & ADL assistance
✔ hydration + nutrition support
✔ medication reminders
✔ fall prevention
✔ family updates + reassurance
