Revive Occupational Therapy is an authorized provider of ADP (Assistive Devices Program) assessments. Through the ADP, eligible Ontario residents can receive funding assistance (75%) for mobility devices such as:
• Walkers (rollators)
• Manual wheelchairs
• Power wheelchairs
• Mobility scooters

We offer fast assessment appointments with reports completed within 1 to 2 weeks!

About Revive

We are a private Occupational Therapy Service Provider for individuals with: physical, mental, learning, and developmental disabilities, illnesses and injuries. At Revive In-Home Occupational Therapy Services, our Occupational Therapists work with adults and seniors in different settings (Long Term Care, Assisted Living, Retirement Home, and individual’s homes) to promote independence in meaningful activities.

Occupational Therapy is a form of therapy that encourages rehabilitation through the performance of activities required in daily life. While physiotherapy focuses on improving mobility and range of motion, our Occupational Therapists have more of a focus on solving problems that interfere with a person’s ability to complete everyday tasks. Our OTs conduct a comprehensive assessment to determine the barriers that are impeding independence.

Assistive Devices Program of Ontario

The Assistive Devices Program (ADP) in Ontario, Canada, is a government-funded initiative designed to help individuals with long-term physical disabilities obtain assistive devices that improve their quality of life and independence. The program provides 75% of funding assistance for a wide range of equipment and supplies, including mobility aids, hearing aids, communication devices, home oxygen, and more.

The ADP aims to reduce the financial burden on individuals and families by subsidizing the cost of assistive devices. It also ensures that individuals have access to devices that meet their specific needs and are of high quality.

To access the ADP, individuals typically need a prescription from a healthcare professional, such as a physician or occupational therapist, outlining the need for the assistive device. The program has eligibility criteria based on factors such as residency, disability status, and financial need.
