The Upper Canada Family Health Team is comprised of a group of health care professionals working collaboratively to provide comprehensive and accessible health care. The goals of the Upper Canada Family Health Team include preventing illness and promoting the health of patients. We aim to reduce the number of patients in Brockville who do not have a health care provider, reduce ER visits for minor illness, and reduce hospital admissions for chronic illnesses.

📢 **Family Day Closure Notice**

All Upper Canada Family Health Team sites and our After-Hours Clinic will be closed on **Monday, February 16, 2026** in recognition of Family Day.

If you need medical assistance while we are closed:
• Call **Health811** at 1-866-797-0007 to speak with a nurse
• Visit Brockville General Hospital’s website for care guidance
• For urgent matters, please go directly to the Emergency Department

We will reopen on **Tuesday, February 17, 2026**.
