The Upper Canada Family Health Team is comprised of a group of health care professionals working collaboratively to provide comprehensive and accessible health care. The goals of the Upper Canada Family Health Team include preventing illness and promoting the health of patients. We aim to reduce the number of patients in Brockville who do not have a health care provider, reduce ER visits for minor illness, and reduce hospital admissions for chronic illnesses.

The UCFHT would like to extend their warm wishes to Dr John Erb as he is now officially retired from his family practice in Lansdowne Ontario. His longstanding dedication to the patients and community will be very much missed and we wish him all the best in his new phase of life.

Taking over his practice is Dr Tara Wakefield, and while she has been working with us for several months now we would like to take this time to officially welcome her to the UCFHT family! We are so pleased for both Dr Erb and Dr Wakefield as they embark on these new adventures!
