High Dose Vitamin C Enhances Immunotherapy (PD-1 Inhibitor) Outcomes

High dose vitamin C enhances immune system profiles and immunotherapy treatment outcomes (PD-1 inhibitors) according to a lung cancer model (mouse). High-dose vitamin C significantly increased CD8+ T cell activity and when administered in combination, significant antitumor effects were observed, and tumor growth was considerably reduced compared with treatment with either anti-PD1 or high-dose vitamin C alone. The high-dose vitamin C increased the infiltration of CD8+ T cells independently of anti-PD1 but increased the killing activity of anti-PD1. This research tends to match our clinic experiences throughout the years alongside checkpoint inhibitors (i.e. Keytruda, Optivo, etc.). There appears to be clinical value in the combination and has also been further supportive from an in integrative model in humans from Austria that incorporated low dose immunotherapy, hyperthermia, and vitamin C.

A single dose of intravenous vitamin C during surgery or as part of the anesthesia process, for total hip replacement resulted in less morphine use, improvement in pain scores, reduction in inflammation, and increased range of motion. The authors felt that it would be advisable in future to examine daily administration of intravenous vitamin C versus looking at the studied 24 hour postoperatively. No signs of problems or interference was reported in this double-blind, placebo-controlled, randomized trial of a 100 patients.

Intravenous vitamin C plus oral maintenance (500mg twice daily) for 6 months may provide value in those who receive bone marrow transplants (allogenic) for blood cancer treatment (i.e. AML, ALL, CML, MDS). It has been reported that blood levels of vitamin C are low in such patients and by correcting the blood levels may further help the body. In this study of 14 patients, overall they did better and there was sign towards lower non-relapse mortality (NMR). While the trial was small, it is nonetheless interesting and tends to mirror clinical observations we have seen throughout the years.

A reminder that even in those who are smokers and stop within 6 months of being diagnosed with cancer can increase their life span significantly. It is not too late to improve quality and time in ones life in smokers who quit. They found that quitting within 9 months also provided value but that 6 months was the ideal window to do so.

Commonly prescribed medications used in cancer care management for pain control (i.e. opiates) and for stomach acid control (i.e. PPIs or proton pump inhibitors) are becoming increasingly linked with worse outcomes in those who receive immunotherapy cancer drug treatment known as check point inhibitors (i.e. Keytruda, Optivo, etc.). While the data is preliminary and retrospective, in clinical practice this area receives little to no attention. You do not hear recommendations for perhaps trying other methods or other medications such as NSAIDs for pain control or the suggestion of the more older anti-acids that may be more benign in nature (I.e. H2 blockers, etc.). It is uncertain if perhaps patients who require such medications may be sicker and hence their immune response more compromised. However, the negative data signal continues to be raised by the growing research. It may be time to have more honest discussions between patients and healthcare providers in such areas as these immune therapy medications are costly and patients deserve optimal responses whenever possible when they are used. Perhaps for some attempting a more lower, but yet effective dose, when using opiates and PPIs should be considered when needed.

The use of NSAIDs helps reduce the risks of the troubling and at times painful Hand-Foot Syndrome in patients using the oral chemo pill Capecitabine (also know as Xeloda). In particular the topical NSAID - Diclofenac Gel was used in this study, where others have also used Celebrex orally with success as well. The topical option worked very well and appeared to be well tolerated on the body. As a result less treatment interruptions due to less raw or painful hands and feet which is a win win.

The consumption of cherries (tart liquid form) - 1 ounce per day (2 tablespoons) mixed in water - improves aches/pains as a result of hormone treatment in breast cancer caused by aromatase inhibitors (i.e. letrozole, arimidex, aromasin). For some woman the discomfort can be quite severe resulting in significant distress where some may stop treatment. Something as simple as cherries, that increases melatonin levels, appears to help in a 6 week research trial.

Fundamentally we are here because of the people with cancer we have helped and the ones we continue to serve

We began as a general naturopathic medical clinic in 1999 that evolved to a clinic focused in treating and supporting people with cancer whether in the early stages of a diagnosis, in active surveillance, during and following treatment, for those living with cancer and wanting more, to those exploring ways to improve survivorship goals, including those interested in exploring early detection methods or other tests of interest, even when considered preliminary or experimental. We have also incorporated technologies more commonly known from other parts of the world so that we could provide such treatments right here in Canada. At times we may be learning together, however, fundamentally we have become experienced in this area of cancer and work with each patient to provide well-informed, experience and research-guided treatment plans whenever possible, using a wide range of innovative approaches along side fundamental supportive care. We have also established positive working relationships with members of the oncology community to access as needed. We are more than just a clinic — we provide a network for empowered patients to support and help each other thrive
