PUPILS!

Aka the window into the autonomic nervous system.

Did you know your pupils are technically controlled by muscles?

The parasympathetic nervous system controls the pupil constrictors (makes the pupils smaller).

The sympathetic nervous system controls the pupil dilators (makes the pupils bigger).

What does it mean then if the pupils suddenly CONSTRICT during an eye movement, like gaze holding? And this patient has long COVID symptoms, including fatigue, brain fog and extreme loss of exercise tolerance?

That would be a failure of the SYMPATHETIC nervous system. They are also lacking sympathetic nervous system output to blood vessels so they aren’t likely getting adequate constriction of blood vessels to support the demand of the muscles during or the brain during cognitive tasks.

Would this be a good patient for ? Actually… no! But when we think , or , we often get stuck thinking we need to do vagus nerve stimulation. This is why exams are so important!

So, here’s the equation for this patient. Question: How do I stimulate her system to encourage adequate sympathetic output?

The answer: Left otolithic and eye exercises that bias activating the right midbrain.

A testimonial from our most recent intensive patient, Melissa, who visited us from Medicine Hat!

Melissa came to us with right sided pain - fatigue and cramping, along with the inability to “turn on” some of her muscles while she was walking. We assessed her with our diagnostics, listened to her history and started piecing together that her right sided body complaints were actually coming from her vestibulospinal pathway.

The vestibular system… AGAIN?!

Yes, again.

Within ONE DAY of combining posterior tibialis stimulation with vestibular rehab, Melissa could walk without thinking about her right foot alignment. Her hip stopped clicking, her knee didn’t hurt and her arch wasn’t collapsing inward. Walking was automatic again, after over 10 years of it being a conscious effort 🤯

Safe to say we freed up a bit of mental capacity for Melissa… and prevented some arthritic changes over the next couple of decades! 🤞🏼

Suffering since 1991…

Swipe left to see the impact of addressing the vestibular system on someone who has been struggling to get doctors to understand her condition since 1991.

Mary is in her 70s now and for the last 30 years has been declining since she slipped and fell down a flight of stairs at work.

She’d been struck by a car as a pedestrian and had two previous MVAs before this fall. The fall was the injury that changed her life, though.

She came to us thinking she had POTS and had been struggling with chronic fatigue for years. But when we did a tilt table test to support a POTS diagnosis we found her heart rate didn’t change more than 10bpm and her blood pressure didn’t change enough either! 😒

We performed the diagnostic tests and on Head Impulse Testing I noticed she really struggled with keeping her eyes on the target when moving her head quickly back/left and forward/right. She didn’t feel “quite right” in the head but it’s how she always feels so at first she reported no change in her symptoms. I became suspicious of polycanal Benign Positional Paroxysmal Vertigo. She also reported things like veering to the right while driving and not noticing it. Or walking into people and not realizing it was her fault. All signs that her vestibular system is not giving her information about where she is in space…

Dr. Ryan performed positional tests this morning and this is the text I received after he found BPPV in her left posterior canal 🥹

I had her for her second appointment of the day and treated right anterior canal BPPV. She stood up and we tested her balance and she didn’t fall straight backward with her eyes closed. For the first time in 30 years she felt GROUNDED.

We still have work to do but now her eyes are stable and we have a stable vestibular system to build upon! 💪🏼

Shutting her down for the long weekend because we need to take a break too.

This is consistently the hardest part for our patients though.

I truly believe everyone WANTS to get better. That usually means people are inspired to be proactive in their recovery or do their exercises, even when their bodies are begging for rest.

If you don’t take the rest, your body will take it for you.

Programming REST into your recovery is key to going at it consistently.

You know those people who can make their eyes jitter?

I've never been able to do that.

But imagine that happening ALL the time and you have no control over it?! 😵‍💫

That's what saccadic oscillations (or ocular flutter) is. It is quick side to side eye movements around a target that the brain is struggling to appropriately fixate on. It can cause issues with visual clarity and make balance and walking really challenging. 😑

In this case, it was caused by vestibular neuritis, which impaired how well the patient was able to detect tilting their head forward and turning it to the right. Which means his eye reflexes, neck reflexes, postural reflexes and foot control were all impaired, too!

By the end of the week, we were able to get his eyes more stable, reduce the severity of his constant dizziness and make him more stable while walking 👍🏼

There is always hope for an improved quality of life, even after years of suffering 😍

New series! - Party trick or pathology?

Hint: all of them will be wild things your eyeballs should not do, unless you have crazy voluntary control over them... in that case, it's your creepy party trick 😵‍💫

Today, we have the Internuclear Ophthalmoplegia (INO).

Your horizontal/lateral eye movements should be linked. You should not be seeing the eyes moving "separately" from one another in different directions. As you can see in the video, this patient is struggling to keep their left eye moving with their right eye when they look quickly to the right 👀

This means that there is damage to the pathway that links the eye muscles together. This pathway is called the Medial Longitudinal Fasciculus. We have two of them, for looking left and looking right, in the brainstem.

For whatever reason, autoimmune processes in Multiple Sclerosis tend to attack THIS pathway first! For board exams, we all had to learn this as the first sign of Multiple Sclerosis.

Fun fact - I have helped patients rule out MS by spinning them in a chair in the dark and observing whether their eyes remained yoked! I receive mixed reviews for this test, as I'm sure you can imagine... 🤢

Using different types of eye and vestibular exercises, we can actually improve the integrity of the MLF and reduce symptoms associated with the eyes not being yoked, like dizziness, eye pain and neck pain! Even if the patient has an autoimmune condition, their nervous systems are still willing to adapt with new exercises.

I despise the phrase “you don’t want to get better.”

When you work with people who have chronic illness, it comes up a lot 😒 Other doctors brush off a patient by saying they don’t want to get better if they can’t figure out how to help them.

Sometimes, the patient hasn’t met the right members of their medical team yet.

And sometimes, the patient is grappling with fear and/or unworthiness. A fear of the responsibility that comes with health. A fear of the responsibility that comes with living more fully. A fear of what comes with co-creating their life during and after recovery. Feeling unworthy of a better quality of life. Feeling unworthy of experiencing a vibrant life.

Who are we to live fully?

Didn’t this trauma/accident/sickness take everything away from me?

Short answer ~ no. It didn’t.
And yes, you deserve to heal.

It may sound odd; but we all deal with some form of this unworthiness, even if we aren’t sick, injured or suffering.

Please give a warm welcome to our newest team member!

This is Dr. Ryan Lawrence (), chiropractor extraordinaire! Dr. Ryan has already had quite an epic experience in the chiropractic world. From serving veterans at the Lee County Veteran Affairs clinic in Florida to working in a multi-disciplinary clinic that utilized in Tennessee, he has learned how to address chronic issues with a lot of different tools and modalities. He fell in love with the functional neurological approach so you'll see him supporting Dr. Rachel's patients, with the goal of being another provider in offering the same unique approach to care!

In the meantime, Dr. Ryan is also available for general chiropractic services for patients with conditions that don't require a functional neurology work up ~ think that annoying low back pain or sprained ankle that need a little TLC.
