South Calgary Primary Care Network

South Calgary Primary Care Network Our Primary Care Network helps physicians collaborate with other providers to enhance care.

The South Calgary Primary Care Network (SCPCN) is a collaborative healthcare organization dedicated to enhancing the health and wellness of our community. Serving the South Calgary area, SCPCN works in partnership with family physicians and healthcare professionals to provide comprehensive, patient-centered care. Our multidisciplinary team offers a wide range of services, including chronic disease management, mental health support, nutrition services, and more. At SCPCN, we are committed to continuous improvement and innovation, striving to meet the evolving needs of our patients through professional development and evidence-based practices. For more information, visit www.scpcn.ca

Our programming is developed and delivered in partnership with family doctors and our wellness partners in the community. Research shows that people who are connected to family doctors and visit them regularly have better health outcomes. In addition, they receive better chronic disease care, make fewer visits to the emergency room and are hospitalized less often.

There are resources and support available to help you or someone you love manage substance use. 🤍

On October 30, Primary Care Networks across Calgary came together for a Truth and Reconciliation event focused on learning, reflection and action.

Through shared stories and dialogue, staff explored their understanding of the systemic barriers and lived experiences that affect access to safe, equitable and culturally grounded primary care for Indigenous peoples.

💛 Together, we continue the work of reconciliation — relationality, responsibility and reciprocity.

Misinformation about mental health can stop people from reaching out for support. Let’s set the record straight.💙

Support looks different for everyone. If you or someone you love is struggling, talk to your primary care provider. They can connect you with the right resources, including a referral to a psychologist or other mental health supports available through our and other programs.

Taking that first step can make a difference.

🔎 What is an invisible disability?

The stranger next to you might look completely fine, but in reality could be managing a mental, physical or neurological condition that impacts their quality of life, like:
· chronic pain or fatigue
· mental health conditions like anxiety or depression
· learning differences
· hearing or vision loss

If you live with a disability, know that your primary care provider and PCN health professionals can support you. Learn more: scpcn.ca
