🤍 This is called Advance Care Planning. You can make sure the people you trust know what to do if there is ever a time that you are unable to make or communicate decisions yourself.

There are lots of free and low-cost community resources to help you with this process, including your health care team. A PCN social worker can help answer your questions and provide you with guidance around completing these documents.
