National Day for Truth and Reconciliation is approaching.

At SOAHAC, this is a time of reflection, remembrance, and commitment to meaningful reconciliation. Our Health Centres will be closed on Tuesday, September 30 to honour this day and its importance to the Indigenous families and communities we serve.

We invite all clients, partners, and community members to wear orange on Tuesday, September 30, a visible act of remembrance for the children who never returned home and a show of support for Survivors and their families.

We encourage everyone to attend a local event, learn from Indigenous voices, and show your support in meaningful, respectful ways.

Let us honour their truths and continue walking the path of reconciliation, together.

We are looking for a Locum Family Physician to join the SOAHAC team at our site located in Cambridge, Ontario. Reporting to the Integrated Care Manager in Cambridge, the Family Physician is responsible for providing comprehensive, patient-centered Primary Care which emphasizes accessibility, health promotion, illness prevention and continuity of care for Indigenous people living off-reserve and surrounding First Nations communities. The physician provides care to individuals across the life span, promoting a wholistic approach to assist individuals, families and groups to achieve an optimal state of health and well-being. This position will require some travel to surrounding First Nations communities.

We're Hiring: Indigenous Patient Navigator in Cambridge, ON

Reporting to the Integrated Care Manager, the Indigenous Patient Navigator is a key resource to Indigenous people, caregivers and their families upon admission to hospital. You will assist Indigenous people with navigating the health care system, ensuring access to and provision of culturally appropriate health care services and link clients with the health and community supports required to transition back to home after discharge and live safely in their homes in partnership with Grand River Hospital and St. Mary’s Hospital. You will also be the lead for Coordinated Care Planning for Waterloo Wellington’s Indigenous clients. This position also acts as a cultural resource for health care providers working with Indigenous people.
