We are looking for a FT Social Worker to join our team

Position Summary
Capreol Nurse Practitioner-Led Clinic (CNPLC) provides health services to the residents of Capreol and surrounding area. The Social Worker works as part of the interdisciplinary team to deliver professional psychosocial services. The Social Worker also develops, implements, monitors and evaluates mental health services offered at the CNPLC and assists with linking clients to appropriate community resources. The Social Worker provides counseling/therapy and other therapeutic modalities for individuals, families and the community.

Key Qualifications
· Bachelor degree in Social Work from a recognized university.
· Master’s degree in Social Work is an asset.
· The Social Worker is licensed with and practices within the scope and according to the standards of practice as outlined by the Ontario College of Social Workers.
· Possess Professional Liability Insurance
· Minimum of three years of counseling experience.
· Ability to travel within Greater Sudbury
· Current Ontario Driver’s License and insurance.
· Ability to work flexible work hours. Some evening work and home visits are required.
· Excellent Communication and organizational skills.
· Demonstrated ability to work independently and in a team environment.
· Demonstrated computer skills
· Possess strong time management skills
· CPIC required

We are looking for a FT Registered Dietitian to join our team

Position Summary

The Registered Dietitian (RD) at the Capreol Nurse Practitioner Led Clinic will directly care for and provide nutritional education to individuals and groups according to best practices. The dietitian will work with an interdisciplinary team and will work in collaboration with team members to optimize plans of care. The dietitian will engage with clients facing a variety of health and wellness challenges including, but not limited to chronic diseases, gastrointestinal issues, metabolic issues and general wellness concerns.

Key Qualifications

· Registered and in good standing with the College of Dietitians of Ontario

· Degree in Human Nutrition and Dietetics from and accredited University.

· Experience in providing educational sessions to groups.

· Experience in providing nutritional counselling with a variety of health and wellness concerns.

· Excellent communication and problem solving skills

· Proficiency in use of computer including email and electronic medical records

· Ability to work independently and within a team

· Ability to work flexible hours and travel within Greater Sudbury as required

· Access to personal vehicle

· Current CPR and First Aid.

Current registration in the Extended Class with College of Nurses of Ontario
Bachelor of Nursing Science Degree (Master’s degree is an asset)
Advanced health assessment and diagnostic reasoning skills
Ability to prioritize, manage time and work in a fast paced environment
Excellent communication and problem solving skills
Proficiency in use of computer including email and electronic medical records
Primary Care experience is an asset
Ability to work independently and within a team
Ability to work flexible hours and travel as required
Access to personal vehicle
