When you’re admitted to a PEI hospital, your care team begins planning the path that best supports your recovery. Sometimes that means being transferred to another Island hospital where the care you need is available. 🌿🏥

Your team will keep you and your loved ones informed, coordinate with the receiving hospital so they’re fully up to speed on your health history, and ensure your care continues smoothly. If you’ve completed the acute phase of treatment, a transfer can help you receive the right level of support in the right place.

For those awaiting long term care, you may be offered the first available bed that meets your needs, with priority given to move you closer to home when a spot opens. 💛

Home Care can provide services in your home, if you:

• need support to continue living safely at home,
• have been released from hospital and are recovering from surgery or an acute medical condition, or
• if you are awaiting a long-term care placement
• have an ongoing health issue requiring more care than you or your family can provide.

Services are provided based on assessed need, for a defined period of time.

A referral can be made by yourself, your family, care provider, or family doctor, and submitted to your local Home Care Office.

Health PEI Patient Navigators are here to help you and your family find the right services, supports, and care, no matter where you are on your health‑care journey.

Patient Navigators can:
✅Connect you with the most appropriate health services and community supports
✅Help you understand how to access care
✅Support you through complex needs, cancer care, stroke recovery, or mental health and addictions services, depending on your situation

While they can’t offer medical advice or change wait times, they can make sure you feel informed, supported, and guided.

Everyone who receives treatment through Health PEI has a health record. 📄

Health records contain information such as reports from care providers, results of tests such as blood work and diagnostic imaging (X-ray, MRI, etc.), and prescribed medications.

To request access to personal health information, you can either
➡ Complete the online form for services at hospitals including blood tests and other lab work and diagnostic tests
➡ Contact the service provider directly
➡ You can now also access your diagnostic imaging reports, laboratory test results and immunization history online through your MyHealthPEI account.
