November 14 is World Diabetes Day: a global event and official United Nation’s World Health Day that unites the global diabetes community. The date was chosen in honour of the birthday of Canadian Sir Frederick Banting, who together with Dr. Charles Best, discovered insulin. https://www.diabetes.ca/

The month of November is celebrated as Diabetes Awareness Month. While significant advances in diabetes management have been made, there is still no cure for those living with diabetes or prediabetes. CKFHT is proud to support an in-house Diabetes Education Program to provide collaborative and supportive ongoing care for patients of CKFHT with a prediabetes or diabetes diagnosis.
Thank you to our wonderful Diabetes Education Team - Heather Butzer (RN, CDE), and Laurie Babin (RD, CDE) – who strive to work with each patient to empower them to self-manage their diabetes on a daily basis!

Funded by the MOHLTC, a Family Health Team is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you - the patient.
Designed to improve and expand access to primary care, most Family Health Teams consist of doctors, nurses, nurse practitioners, social workers, dietitians and other health care professionals who work collaboratively, each utilizing their experience and skills so that you receive the very best care, when you need it, as close to home as possible.
The CKFHT is not a walk-in clinic and only patients who are rostered to the physicians of the CKFHT have access to the programs and services of the CKFHT. This is the same for all Family Health Teams across the province.
There are currently over 3 million Ontarians enrolled in Family Health Teams in over 200 communities across Ontario.
The CKFHT has 5 sites across the Municipality of Chatham-Kent – Chatham, Ridgetown, Blenheim, Wallaceburg and Dresden.
