Thamesview Family Health Team provides primary health care, including chronic disease management and health promotion and prevention, to a rostered population of patients in the Chatham-Kent region. The FHT utilizes an interdisciplinary team of health care professionals who work in a collaborative model of care. Currently, our team is made up of the following:

Our NPs at the Thamesview Family Health Team have openings TOMORROW, Wednesday February 17th from 3:00 PM - 5:40 PM.

If you received your letter in the mail advising you're due, or turned 25 and have yet had a pap smear, call and get booked!

📞 ROSTERED PATIENTS: If you are a rostered patient at Thamesview Family Health Team, call your doctors office to inquire about booking.

📞 UNNATTACHED PATIENTS: If you do not have a primary care provider, call us directly to book - 519-354-0070 (press 2, then ext. 602). If this date and time do not work for you, please call 519-397-5455 x 159 to schedule into the monthly Pap Clinic offered by the primary care teams across Chatham-Kent.
