Patient Connect NL is a provincial list of individuals who have identified as being without a Primary Care Provider (Family Doctor or Nurse Practitioner) in Newfoundland and Labrador (NL).

Individuals may register online or by telephone (1-833-913-4679) to be placed on the list, with the goal of becoming a patient of a Collaborative Team Clinic (CTC) and attached to a Primary Care Provider.

Our current supply of Moderna vaccine is completely booked up. We are working diligently to secure more vaccine when it becomes available from the Provincial Government. We hope to resume booking appointments early in the New Year!

Due to circumstances beyond our control we will not be able to provide Covid-19 Vaccinations this Thursday and Friday (Dec 2nd and 3rd). The supply of Vaccine from Eastern Health may be made available to us next week. Please call ahead to book, rebook or verify your appointment.
