Registered Nurse – Primary Care & Seniors Health

Are you a compassionate, collaborative Registered Nurse who thrives in a team-based environment and wants to make a meaningful impact in rural healthcare? The Consort Medical Clinic is expanding our integrated primary care team and is seeking an RN with a passion for seniors’ health, chronic disease management, and primary care, in a team-based model.
This position plays a key role in enhancing access to primary care, supporting our senior and geriatric population, and strengthening our Nurse Practitioner-led model of care.

What You’ll Do
You will work closely with our Nurse Practitioners to deliver patient-centered, whole-person care through responsibilities such as:
Clinical Care
• Conduct patient intake assessments and provide teaching/coaching
• Support triage and same-day appointment needs, including walk-in patients
• Provide chronic disease management (diabetes, hypertension, COPD, etc.)
• Deliver preventive screening, and health promotion
• Support our Seniors Health NP with EMR tasks and panel management
Coordination & Panel Management
• Support EMR-driven population health initiatives
• Participate in panel management and care-gap closure
• Assist with referrals, follow-up, and care coordination for seniors
Team Collaboration
• Work closely with NPs, MOAs, and allied health partners
• Contribute to a culture of quality improvement and patient safety
• Perform additional duties as assigned by the Nurse Practitioners

What We’re Looking For
• Registered Nurse in good standing with the College of Registered Nurses of Alberta (CRNA)
• Experience in primary care, community health, geriatrics, or chronic disease management (asset, not required)
• Strong clinical judgment, communication skills, and patient-teaching abilities
• Comfort working both independently and collaboratively within a fast-paced team
• Commitment to rural health, accessibility, and patient-centered care
