WHAT IS AN ARM LIFT?
“Brachioplasty surgery treats sagging skin under the upper arm by removing excess tissue and tightening and smoothing the remaining skin. This reshapes a more youthful, contoured and defined looking arm that is proportionate to the body,” explains Dr. Robert Wilke, a Board Certified Plastic Surgeon with Edina Plastic Surgery. “Our upper arm skin can sag due to simple aging and genetics, but as surgeons, we most often perform Arm Lift Surgery on patients who have undergone a substantial weight loss. While exercise can build the underlying muscle and supply some definition, if the overlying skin has lost its elasticity, the arms will never look tight and smooth without surgical intervention.”

There are several ways of performing Arm Lift Surgery. These are dependent upon the amount of excess skin present and where it is. The most common Brachioplasties involve incisions made to the inside or backs of your upper arms. Other techniques include an incision made in your armpit that extends all the way to your elbow. The goal is to safely remove as much excess skin as possible, recontour the arm, and to keep scarring discreet and at a minimum.

“There are scars involved with Arm Lift Surgery, but most patients find the end result well worth the trade-off,” says Dr. Wilke. “To comfortably wear a tank top again and simply find clothes that fit in the arms the right way is a huge reward to the patients I work with.”

LIPOSUCTION AND ARM LIFT SURGERY
“Sometimes, I consult with patients concerned about the appearance of their arms who assume that liposuction is all they need,” says Dr. Wilke. “While liposuction is effective at removing excess deposits of fat from the upper arm, it does nothing to tighten the sagging skin in that area,” he explains. “In fact, liposuction without a subsequent Arm Lift can make the upper arms appear even more droopy and deflated. In cases where there are thickness and fat to the upper arm and sagging, we may opt to do both liposuction and Arm Lift Surgery during one operation. If the skin is thin, crepey, and deflated, an Arm Lift may be all that is needed.

WILL I HAVE SCARS FROM ARM LIFT SURGERY?
“Yes. You will have scars following your Arm Lift Surgery,” says Dr. Wilke. “With proper post-care, scars will be less noticeable over time, but there is no way to surgically remove excess skin in this area without creating incisions that lead to scars.”

Scarring is a significant consideration for many people contemplating having Brachioplasty surgery. While a short-scar Arm Lift Surgery exists, this is limited only to patients whose loose skin is to the posterior of the arm close to the armpit. In this case, a scar can be easily concealed.

A traditional Brachioplasty surgery scar extends from the armpit to the elbow either on the inside of the arm, or towards the very back. This incision can make the scar somewhat more challenging to hide. Regardless, scars that heal well significantly fade over time, especially when they are protected from sun exposure after surgery.

WHAT RESULTS CAN I EXPECT WITH ARM LIFT SURGERY?
Although visible improvements are seen immediately after Brachioplasty surgery, swelling and bruising must be allowed to subside before assessing your final results. During recovery, you will wear compression garments, and some movements may be restricted due to your healing sutures. The stronger and healthier you go into surgery, the better your results!

“It’s natural for our skin to lose firmness as we age, but the results you get from Arm Lift Surgery are relatively permanent if you maintain a healthy lifestyle,” shares Dr. Wilke. “This is such a satisfying surgery to perform because patients are ecstatic with their results and often comment on how they had to go out and buy an entirely new Summer wardrobe that showed off their arms again!”

REMOVING EXCESS SKIN AFTER WEIGHT LOSS
Losing weight can transform your health and overall life. The rewards of reducing your Body Mass Index (BMI) extend well beyond a decrease in blood pressure and risk of cardiovascular disease; they can also include a major boost to your self-confidence! But, some unwanted side effects frequently mirror the results of significant weight loss. Sagging, deflated excess skin on the body and face is a common concern of many people who lose a large percentage of their body weight. Plastic surgery provides effective solutions to eliminate this excess skin and allow patients to fully realize the rewards of their weight loss victory!

“Tummy tuck is one of the most common surgeries we perform at Edina Plastic Surgery,” shares Dr. Philbin. “This surgery’s appeal is vast, versatile, and varied, ranging from mothers who want to address sagging abdominal skin after pregnancy, to men looking to restore a more youthful-looking waistline after weight loss. Abdominoplasty is customized to each patient’s needs and involves the removal of excess skin (and sometimes additional fat with liposuction) from the abdominal area to create a smoother and firmer abdominal profile.”

ARM LIFT (BRACHIAPLASTY)
Loose, crepey, sagging skin behind the upper arms is a common complaint of those who have lost significant weight. Arm lift surgery addresses this area by creating an incision between the underarm and elbow area and removing excess skin. The result is an upper arm with smoother skin and a more contoured and toned appearance.

BREAST LIFT (MASTOPEXY)
“The female breast is largely comprised of fat,” explains Dr. Philbin. “A women’s weight loss journey may be accompanied by a decrease in the size of her breasts and an undesirable deflation and sagging of breast tissue. Breast lift surgery can raise and reshape the breasts after weight loss. Breast implants may also be used to replace lost volume and to enhance overall firmness and shape.”

The goal with this procedure is to lift and tighten the skin of the abdominals, thigh area, and buttocks – the circumference of the body. The scars that result from body lift surgery are customized to match the patient’s natural contours, and the surgery itself is tailored to match the specific needs and goals of each patient.”
FACELIFT
Many of us don’t anticipate the changes that significant weight loss can lead to in our faces. Unfortunately, we can’t select where on our bodies we’d like to lose fat when we embark on a weight loss journey. Fat loss in the face can lead to loss of support and result in drooping skin around the jawline and a “turkey neck.” So, even though we maybe feel more energetic and youthful than ever after our weight loss, our face appears older and more tired-looking than it did before. Facelift surgery can effectively address these changes by removing excess skin and tightening our jawline and neckline to a more youthful positioning. The use of facial fat grafting may also be recommended to restore some of the desirable volume that fat once provided.

5 WAYS TO FADE & TREAT YOUR TUMMY TUCK SCAR
According to the Plastic Surgery Statistics Report, more than 123,000 Americans decided to trim and smooth their waistlines with Tummy Tuck Surgery (abdominoplasty) in 2019. Tummy Tucks are consistently one of the most requested plastic surgeries from year to year. Despite its popularity and high patient satisfaction, abdominoplasty does present a drawback for some people in the form of a significant scar.

“A Tummy Tuck has a dual focus. It removes excess skin and fat after weight loss or pregnancy, and corrects abdominal muscle laxity,” explains Dr. Robert Wilke, a board-certified plastic surgeon with Edina Plastic Surgery. “Because quite a bit of skin is typically removed, a long incision is required that will result in a scar after surgery.”
When your plastic surgeon assesses your abdominal area for Tummy Tuck Surgery, he or she is looking at the skin from the bottom of your rib cage to the crease of your groin. Generally, the scar that is left behind after surgery sits very low across the abdomen. This makes it easy to hide beneath the waistline of clothing or a bathing suit.

“How your skin scars after surgery is very dependent upon the skill of your surgeon, your genetics, and how you care for your scar after your procedure,” says Dr. Wilke. “A few weeks after surgery, after your skin has healed, patients can begin to use products at home that help lighten and fade their Tummy Tuck scar. After a few months, you can begin exploring in-office treatments that can make your scar even less noticeable. THE MOST EFFECTIVE TREATMENTS TO FADE TUMMY TUCK SCARS
Abdominoplasty scars can be quite noticeable for up to a year following your procedure. Scars on lighter skin types generally turn pink first and then fade to a thin, white line. Darker skin types may heal dark and pigmented at first and then lighten over time. Although this scar will never entirely go away, here are some of the most popular and effective treatment options to minimize its appearance.

#1 – Taping


“We typically close abdominoplasty incisions with dissolvable sutures placed under the skin,” explains Dr. Wilke. “We then cover the incision with steri-strips/paper tape, which protects the incision and minimizes tension. The light pressure from the tape against the incision has been shown to help minimize scarring as well. We often have patients continue to tape the incision for about 6 weeks; changing the tape every 5-7 days.”

#2 – Scar Fading Creams & Topicals
“There are many scar creams on the market,” says Dr. Wilke. “None of them are a miracle cure for scarring, but, nonetheless, they can be helpful to minimize a scar. The simplest and least expensive option is using moisturizing cream with vitamin E. Some of the more advanced creams contain silicone, and some have hydroquinone or other skin lightening agents to suppress pigment cell activity. We usually don’t recommend using these creams until about 6 weeks after surgery.”
#3 – Laser and Light-Based Treatments


How your Tummy Tuck scar appears a year after surgery is, most likely, how it will remain without the aid of additional treatments. This is when you can begin exploring options like resurfacing lasers or intense light-based treatments to pull any pigment from the scar or fade residual redness.

“Laser resurfacing treatments like HALO™ and Profractional can make scars less noticeable by smoothing their texture,” says Dr. Wilke. “Light-based treatments like BBL (Broadband Light) target darker pigment cells, which can lighten the coloration of a scar and reduce redness. In some cases, both treatments may be beneficial, but it’s important to realize that you will need multiple treatments to see significant improvement. At Edina Plastic Surgery, we believe in an all-inclusive approach to surgery that addresses our patients needs from start to finish. We’ve created a Post-Surgical Scar Treatment package to ensure that our patients have the best opportunity to heal with minimal scarring after any surgery we perform. This package includes a series of 6 BBL and Profractional laser resurfacing treatments performed 4-6 weeks apart as well as Stratamed Scar Recovery Gel.”

#4 – Microneedling Collagen Induction Therapy


Microneedling devices, like SkinPen®, use tiny surgical-grade needles to create thousands of micro-injuries to your skin. In turn, this stimulates your skin’s collagen and elastin production, and it heals firmer and smoother. This treatment can be especially beneficial for scars that have healed unevenly. A series of microneedling treatments can result in a soft, flat Tummy Tuck scar that blends very well into the surrounding skin.

#5 – Permanent Cosmetics or Tattoo


“This is thinking outside of the box, but I have had patients who eventually choose to cover their Tummy Tuck scar with a tattoo,” says Dr. Wilke. “This is obviously not for everyone, but because the scar sits so low on the abdomen, it AND the tattoo can be easily covered. Patients will need to wait until their scar is fully healed, about a year, before they can have a tattoo.”

In recent years, the Permanent Cosmetics industry has begun to provide options to disguise surgical scarring. This technique involves your provider using a flesh-colored ink that is closely matched to the skin surrounding your scar and essentially “filling in” your scar. It is crucial to not only go to a reputable provider for this treatment but also to avoid tanning the skin near your scar, as the scar will not achieve the tan of the surrounding skin.

“As surgeons, we do everything we can during your surgery to minimize the appearance of your future Tummy Tuck scar,” says Dr. Wilke. “An experienced surgeon will avoid making your incision too high, and strive to make it as small and symmetrical as possible without sacrificing your results. To date, I have never had a patient who didn’t think that the rewards of their Tummy Tuck far outweighed the scar!”

PLASTIC SURGERY IN THE PRESS – THE “DEEP PLANE” FACELIFT
If watching any of BRAVO Network’s “Real Housewives” shows is one of your guilty pleasures, you are probably in-the-know about cast member Sonja Morgan’s recent Deep Plane Facelift. She and almost every celebrity news facet have inundated the internet with her before and after images. And the truth is – the pictures ARE impressive. But, the heightened popularity of the procedure has many wondering how it differs from a traditional Facelift and whether or not the procedure is safe. Let’s dive deeper into the details behind the Deep Plane Facelift and why many surgeons still gravitate towards a more traditional route for patient safety and results.
“Celebrity plastic surgery on TV, magazine, and social media is a blessing and a curse to your everyday plastic surgeon,” says Dr. Michael Philbin with a smile.

As a Board-Certified Plastic Surgeon at Plastic Surgery, he has performed hundreds of consults that include patients’ magazine clippings or smartphone pictures of reality tv stars and Hollywood actors after their plastic surgery ‘tell-alls.” “In their best light, celebrity’s surgeries draw attention to some of the wonderful things that our specialty can achieve to enrich people’s lives. But, some of these procedures can also sensationalize irresponsible approaches to surgery, unsafe techniques, and truly misguide the everyday person. It’s important to educate yourself about the procedure you are interested in from credible resources (no E!News) and to discuss your options with a plastic surgeon before deciding what is right for you.”

WHAT IS A TRADITIONAL, OR SMAS FACELIFT?
Officially named by surgical anatomists in the 1970s, the Superficial Musculo Aponeurotic System, or SMAS is a dense connective tissue layer within our cheeks. It lies between our subcutaneous fat above and the muscles that control our facial expressions below.
“The earliest Facelifts performed were truly single layer procedures,” explains Dr. Philbin. “Because they only tightened the skin, a patient experience good results for only a few years and might have suffered from poor scarring due to high tension on sutures. Surgeons eventually began to tighten the SMAS layer to enhance the lifting capability, which also improved the longevity of results and dramatically improved scarring due to ease of tension. Tightening the skin will always be an integral part of a Facelift, but true “lift” comes from that SMAS effect.”

WHAT IS A “DEEP PLANE” FACELIFT?
The “Deep Plane” Facelift was developed in the 1990s by Dr. Sam Hamra, a plastic surgeon from Texas. With this technique, the skin and SMAS are raised and tightened as a single layer. Some surgeons believe that this method provides better circulation for the overlying skin. In contrast, others show concern that the technique poses a greater risk for facial nerve injury due to the depth at which it is performed.

HOW DOES A SMAS FACELIFT DIFFER FROM A DEEP PLANE FACELIFT?
“In simple terms, a SMAS Facelift raises the SMAS and skin layers separately, while the Deep Plane does it together,” explains Dr. Philbin. “One huge advantage of the SMAS, or traditional Facelift, is that it can tighten your skin in different directions. From my perspective, as your surgeon, this affords me better control and freedom to customize your results. The Deep Plan method limits your surgeon to one direction of tightening.”
As Dr. Philbin mentions, the SMAS Facelift is a more versatile technique. Not only does it lift the SMAS as a separate layer, but your surgeon may modify it as well.

For example, if a patient presents very heavy jowling in the low face, a surgeon may actually excise part of the SMAS layer or fold it on itself. They may also choose to move the SMAS layer more inward towards the midline of the face if a patient has significant sagging in the central cheek. Regardless of individual face shape, bone structure, soft tissue volume, or skin quality, the Deep Plane Facelift can only perform the same action for every patient.

IS ONE TYPE OF FACELIFT SUPERIOR TO THE OTHER?
“At Plastic Surgery, we favor the Traditional, or SMAS Facelift for its proven efficacy and safety, and because we believe it delivers superior results,” says Dr. Philbin. “No two faces are the same. The SMAS Facelift allows your surgeon to restore a more youthful and natural-looking appearance to your face because it can be customized. We believe that the Deep Plane Facelift’s deeper nature may pose a greater risk of facial nerve injury. With no clinical studies or evidence pointing to patients getting a more natural or superior result with the Deep Plane technique, we do not think this is a risk worth taking for our patients.”

6 THINGS YOU NEED TO KNOW ABOUT NECK LIFT SURGERY
Have you been taking greater notice of your neckline lately? If so, you’re not alone. Millions of Americans are now working from home and staring down at their reflections during countless ZOOM meetings and video conferences. This has prompted many of us to become a little more tuned in with our appearance and where and how our faces may be aging. #1 – YOUR NECKLINE IS ESPECIALLY SENSITIVE TO THE AGING PROCESS.
Although most of our anti-aging efforts tend to be focused on our faces, our necks are especially vulnerable to the passing of time. SHOULD I LOSE WEIGHT BEFORE PLASTIC SURGERY?
Contrary to popular belief, plastic surgery cannot solve every issue you may have with your appearance. Although every plastic surgeon wishes that he or she could wave a magic wand and make any problems patients have with their weight disappear, the truth is that most of them will encourage you to be close to or at your ideal weight before surgery. Why? Some surgeries flat out can’t safely or effectively be performed if your Body Mass Index (BMI) or weight exceeds a certain amount. There are also significant benefits to being as fit and healthy as you can be before your procedure. Let’s take a look at what those are to understand better how you can set yourself up for a successful procedure in the future!
Most of us consider plastic surgery in the first place because we want to improve, enhance, or alter some aspect of our face or body. But, it’s important to appreciate that if you want the best results, you’ll need to put in some work before surgery if you’re currently overweight. Although the numbers on the scale may go down after some plastic surgeries, it should never be viewed as a weight-loss method.

For example, tummy tuck (abdominoplasty) surgery is one of the most popular procedures performed worldwide. If you are considering the right implant size for your breast augmentation, or you are interested in breast lift surgery, you’ll want to be at your healthiest possible weight and one that you can maintain.
Excess weight not only puts a strain on our bones and joints; it also affects our skin. If you are healing from a surgery with a long incision, like a tummy tuck, you’ll want as little strain as possible pulling along your sutures. Being in a healthy weight range before surgery can improve how your skin heals around an incision. In addition, being in great physical shape before your surgery improves all other aspects of your recovery and may even decrease your downtime. Being at a healthy weight also reduces your risk of certain complications like infection, incision healing problems, and blood clots.
It’s important not to be discouraged if your plastic surgeon recommends that you lose weight during your initial consultation. Realize that they want you to get the best results from your surgery too! This means proceeding as safely as possible and respecting the limitations of some surgeries regarding a patients’ body weight. It also means anticipating your health and comfort during recovery. An experienced and credible plastic surgeon will always recommend that you be in the best state of health possible before embarking on your surgical journey. They want this to ensure that you get the best surgical results and because they care about YOU as an individual and know the benefits that being at a healthy weight can bring to all aspects of your surgery.
