I’m excited to say that I recently earned my certification as a Medical Level 2 provider. Whether you are new to the sport or have been playing all your life, working closely with a TPI certified practitioner can help you achieve or maintain pain-free longevity in your golf game, while pushing the limits of performance. TPI looks at how the human body functions in relation to an efficient golf swing. SCIATICA⚡️😫

Sciatica is a form of nerve pain characterized by sharp, shooting, electrical-type pain into the back of the lower extremity (buttock, thigh, calf, etc.). This irritation of the sciatic nerve may coincide with lower back pain and stiffness. The leg pain may be accompanied by numbness or tingling in areas of skin, or weakness in muscles innervated by the sciatic nerve, its divisions, or the spinal nerve roots that combine to form it.
 
The symptoms may be due to an issue in the lower back, such as a lumbar intervertebral disc injury, which will likely be accompanied by back pain. The leg pain experienced is typically more severe than the back pain and is a result of irritation to the previously mentioned nerve roots. Alternatively, the nerve could be compressed/irritated along its path down the back of the leg, most commonly by the piriformis muscle at the hip. The sciatic nerve passes just underneath this small muscle, so excessive tension can lead to compression. This is known as piriformis syndrome where there is often no back pain, rather a deep pain felt in the buttock region. This form of sciatica is common in those who sit for large portions of the day and are lacking hip mobility!
 
Management of sciatica will vary depending on the mechanism and severity of symptoms. No matter the cause, it is key to relieve the compression/irritation and restore mobility to the neural tissue. A detailed history and physical exam help determine what is driving the sciatica with the initial goal of ruling in or out the spine as the main culprit. A large majority of sciatica is a result of lumbar disc injury, where we’ll first look for movements of the low back that reduce the leg pain (directional preference). With piriformis syndrome, the goal is to reduce tension within the external rotators of the hip. With both these conditions, manual therapy such as soft tissue work and adjustments help improve range of motion and decrease sensitivity of the sciatic nerve. Long-term conservative management of these conditions will involve rehabilitative exercises with the goal of improving torso stability, loading mechanics, and addressing movement deficits. 💪🏼

Do you deal with stiffness between your shoulder blades that you just can’t seem to stretch out? Or do you have a hard time lifting overhead at the gym? Most of us could benefit from taking steps towards improving our mid-back, or thoracic spine mobility!

The thoracic spine consists of 12 spinal vertebrae which connect to the rib cage (shown above in blue). It can be thought of as a transitional region between the more mobile cervical and lumbar areas. The mid-back plays a big part in torso mechanics, most notably with rotation, flexion and extension movements.

Most of us spend a large portion of our waking hours sitting and not moving this area through its full range of motion which contributes to it becoming tight and restricted! Adequate movement and motor control through the T-spine is important for everyday activities and sport performance. So, whether you are looking to be able to reach into the back seat of your car with ease or improve your golf swing for next season, look to the thoracic spine!

Swipe for some movements you can incorporate into your warmup routine before exercise or to help break up your workday!

Start with 2-3 sets of 10 repetitions of each exercise.
1️⃣Open Book: Side Lying Thoracic Rotation:
Head supported. Bottom leg straight. Top leg pressed lightly into the foam roller (stay there throughout). Follow your thumb with your eyes.
2️⃣Cat-Cow/Camel: Flexion & Extension:
Inhale as your belly button drops towards the floor. Exhale as your spine reaches for the ceiling. Use your head/neck and low back/pelvis to drive further ROM through the thoracic spine. This is a great full spine mobility exercise!
3️⃣Segmental Cat-Cow/Camel:
Slowly move into spinal flexion starting with your neck, then try to progressively move segment by segment towards the tailbone. Repeat but in reverse, taking about 10 seconds in each direction. This can be quite tricky but is great for improving motor control of your spine!
4️⃣Standing Thoracic Extension with Foam Roller:
Tuck the ribcage down. Allow your body to drop slightly towards the wall as your arms go overhead on the foam roller.
