Many people are surprised to learn that you don’t actually need ovarian cysts to be diagnosed with PCOS. In fact, the name can be a little misleading. Diagnosis is based on what’s called the Rotterdam Criteria, which means you need to meet 2 out of 3 markers: irregular or absent ovulation, signs of excess androgens (like acne, excess hair growth, or elevated hormone levels), and polycystic ovaries seen on ultrasound. So even if your ovaries look “normal,” you can still have PCOS if you meet the other two criteria.
