🟡 Ongoing Monitoring & Guided Support: For individuals who benefit from regular RN follow-up, symptom tracking, functional observations, and structured monitoring.
🟠 Coordinated Complex Support: For individuals with multiple or more complex needs that can be safely supported at home or in the community through close nursing oversight and integrated care planning.
