**Job Description:**
We are seeking a compassionate and dedicated Home and Community Care Worker to join our team. The successful candidate will provide essential care and support to clients in their homes, helping them with daily activities and ensuring their well-being. This role is vital in maintaining the health, safety, and comfort of our clients.
**Responsibilities:**
- Assist clients with personal care tasks such as bathing, dressing, grooming, and toileting.
- Provide mobility assistance, including transferring and positioning clients.
- Assist with meal preparation, feeding, and monitoring dietary needs.
- Perform light housekeeping duties, including cleaning and laundry.
- Administer medications as prescribed and monitor for side effects.
- Accompany clients to medical appointments, errands, and social activities.
- Provide companionship and emotional support to clients.
- Monitor and report changes in clients' health and well-being to the healthcare team.
- Maintain accurate and up-to-date client records.
**Qualifications:**
- High school diploma or equivalent.
- Certification in Home Health Aide (HHA), Personal Support Worker (PSW), or equivalent preferred.
- Previous experience in home and community care or a related field is an asset.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Reliable transportation and a valid driver’s license.
- Ability to handle physically demanding tasks.
- Compassionate, patient, and empathetic demeanor.
