Great conversations at the Seniors Information & Active Living Fair!

Members of the SEMEL Ontario Health Team (Sudbury, Espanola, Manitoulin, and Elliott Lake) were happy to attend the Seniors Information & Active Living Fair on Friday, March 6 at Parkside in Sudbury.

Our team hosted a booth where we spoke with many community members about accessing primary care and helped ensure people knew how to register for Healthcare Connect if they do not currently have a family doctor or nurse practitioner.

We were also grateful for the opportunity to present at the event and share more about the work happening across the Ontario Health Team, including:
• Patient attachment initiatives
• The importance of Healthcare Connect
• Our COPD project
• Future work on CHF and dementia care pathways
• Our Persons with Lived Experience Council
• How we work together as an Ontario Health Team

We also met several community members who were interested in becoming involved with our Persons with Lived Experience Council, helping ensure patient voices are part of shaping the healthcare system.

A great evening connecting with primary care partners in Sudbury!

On Monday, March 2, the SEMEL Ontario Health Team (Sudbury, Espanola, Manitoulin, and Elliott Lake) hosted a Primary Care Network (PCN) event at Natura in Sudbury, bringing together providers and partners from across the region.

One of the highlights of the evening was hearing from local providers who received Ontario Health Team funding, who shared how they have used that support to help attach more patients to primary care.
We also reviewed the SEMEL Ontario Health Team’s digital health roadmap and plans to move this work forward over the next year.

In addition, a guest speaker joined us to discuss central intake, including the progress being made and the next steps for advancing this work across the region.
It was a terrific evening of conversation, collaboration, and shared learning. Thank you to everyone who joined us and contributed to the discussion.

We’re pleased to offer preventative care appointments for unrostered patients at the Elliot Lake Family Health Team. These visits may include preventative health assessments, cancer screening, chronic disease management, prescription renewals, and immunizations.

Appointments are available on Wednesday, January 21 and Monday, January 26.
