The required vertical bone height in the posterior maxilla is acquired, created and grown using the lateral wall accessed external sinus augmentation procedure. Many patients that have a loss of posterior maxillary teeth will undergo a process of pneumatization ! The dropping of the original sinus floor towards the crest of the patients edentulous ridge. To off set. The use of a mixture of the patients bone. Harvested during access to the internal aspect of the maxillary sinus and human donor bone and xenograft. In this case. Cow bone. Time of healing is dependent on volume of the graft. In this case. Min. 9 months to 1 year! Upper left side performed 5 years previously. grafts #

Implant placed 16 years ago on patient. Patient doesn’t come back for crown until 16 plus years later. Tissue in area flat. No papilla present. Expose implant. Do a split palatal crestal flap rolling connective tissues from palatal side of crest to facial. Gives us volume of hood thick tissue to work with. Start tissue engineering. Immature papilla present only because tissue I move to facial ( lip side) is thick and robust. Place provisional plastic screw retained crown and adjust every 3 to 4 weeks to allow creeping of tissue between teeth. “Pink triangles” start to form. Two months. Four months. Then aesthetic demand patient only moderate. Decide on soft tissue impression. Place final crown screw retained. Papilla(pink triangle) will continue to develop. My longest in between implant placement and prosthetic ( crown on implant) in my 36 years practise. But respecting soft tissue and allowing for good thickness and robust blood supply and excellent lab work (Markus Michael Master Technician , owner Full screen dental lab we get good end results. Working as a team. Patient, Soft tissue surgeon and prosthetic development with lab. Allows us a happy patient. You know the saying. “ It’s NEVER too Late!”

Not Always Implants in the jaw to solve problems. Patient history of multiple crown and bridge procedures. Ptx age 78 yrs old. Ask “we can remove front four lower teeth and place two implants. Will take months before completed . Or we remove. Do large double abutment bridge , fixated to two teeth either side. Let sure heal with temporary bridge for one month. Prep teeth supra gingival. All kinds of axial walls to retain cemented bridge. Done in two weeks ! Patient states. “Doc, I thought you were going to talk me into implants. !”
Me. “ Sir, there are multiple ways to provide sound long lasting dental treatment. We offer and give advice! Me sir. I do the bridge on myself if I were you. Great result. Patient happy with shade of 8 teeth. To match upper bridge. Think of multiple treatment plans to serve your referral.

Everything Right! How can it go So Wrong!Statement”Doc, I thought I could bite into the sun with the huge hunk of cheddar cheese slab in it with my side teeth!”😂😞!Patient presents back in a few weeks. Broken sutures. 6-0 prolene. Torn through tissue. Great way to start Monday. They ask” What did you do wrong?”Then they fess up! And it’s. “Doc, what can we do to make up for my F- - k Up?At this point , even though all kinds of answers come into your mind that is racing. But sometimes the answer is not what can be done. But what Not To Do!Sometimes less is more! Wait ! If we are getting some type of heal. And we have good keratinized gingival tissue. And not just loose mucosa. The body is a machine. Stop. Wait. See patient weekly. See where it goes. You can always. Do this or that ! But let Mother Nature intervene! She’s an Amazing Lady. This time I did. Six months goes by. CBCT scan taken. Large nasal palatine foramen! Hindsight. May have grafted. But did sockets only with Hard/soft tissue augmentation! But. Patient has diastema ( space ) between two anterior natural teeth. Do implant position such. Great result. Sometimes. LESS IS MORE! Thought for THE DAY!

The power or guided bone regeneration. Failing lateral maxillary incisor. Root canal treated. Removal shows facial bone defect. Need to follow the P.A.S.S.principles.Achieve primary closure.Provide angiogenesis (blood to area).Space formation and maintain for bone in growth.Stability of the wound site to facilitate blood clot formation and healing. Bone tacs placed support to site to stabilize the bioguide membrane. Mixture of xenograft/ allograft to allow volume and long term matrix / scaffolding of graft for stability. Re-enter site 9 months post grafting to allow for horizontal bone growth and re-establishment of facial bone contour and form!
Procedure so easy to follow, but sometimes not follows precisely. Which leads to instability of membrane. Then lack of space maintenance. As well not a passive closure of site to allow primary closure. Improper release of flap vertically one cause. Periosteal scoring needed. With split thickness advancement. Vertical release incisions at least one tooth away on Borge sides of graft site, allows for prevention of opening and bacterial invasion. If protocol followed. Get excellent results. As we did here. To allow for proper positions of implant and direction of screw access out through cingulum of the crown.
