October 1 is National Seniors' Day!

We believe that healthy aging isn’t just a goal for October, but a lifelong journey worth celebrating every day. That’s why we're proud to offer a free Healthy Aging Workshop Series, designed for seniors and their caregivers. These are free, one-hour workshops that are held in-person and virtually.

Each session is facilitated by healthcare professionals such as registered dietitians, social workers, pharmacists and physiotherapists, who understand the unique needs of older adults and offer compassionate, expert guidance.

Come walk with us and learn some tips for mindful living! 🚶

Join us at Bayfront Park tomorrow, Friday July 25, from 9:00 - 10:00 a.m. for another Just Walk event, powered by : a free, one-hour walking program designed to bring the community together while promoting better health.

Tomorrow's session, hosted by Tricia Brinn, MSW, RSW and Mental Health Groups Coordinator, will be all about mindful ways to manage stress. Check out this tip from Tricia and we hope to see you there tomorrow!

"Wellness doesn’t just have to be hour-long meditations or complex routines. Consider weaving micro-practices into your day—brief, intentional pauses that can support your well-being over time. These might include a few mindful moments while walking, sipping your coffee, eating, showering, doodling, or even doing dishes. Small shifts in everyday habits can create powerful changes in how we feel."

At the Hamilton Family Health Team (HFHT), we know how important it is to create safe, affirming spaces for 2SLGBTQIA+ communities—not just during Pride Month, but all year long. 🌈🏳️‍⚧️

In honour of Pride, we’re highlighting two of our free programs: AFFIRM and our Gender-Affirming Care workshop. Both offerings reflect HFHT’s ongoing commitment to equity, accessibility, and patient-centred care.

Today is National Nurses Day, marking the birthday of Florence Nightengale and the start of National Nurses Week! 🎉

This year’s theme for Nursing Week is The Power of Nurses to Transform Health.

National Nursing Week serves as an opportunity to recognize and celebrate the expertise, dedication, hard work and achievements of all Registered Practical Nurses, Registered Nurses and Nurse Practitioners in Hamilton, across Canada and around the world. (CNA, 2025)

Primary Care Nurses have impact on the lives of many people in our community. Through their care, compassion, and expertise, they contribute to positive changes in the health and well-being of individuals throughout the various stages of life. Having nurses in primary care practices allows for expanded team-based care, which allows more people to have a primary care home and better access to a family physician.

This Nursing Week, the Hamilton Family Health Team would like to express our deepest gratitude for your dedication, compassion, and selflessness to your patients and your profession. You are an inspiration to us all.

Introducing: Our New Strategic Plan, 2025-2028! 🎉

Leading the way to a healthier community through team-based primary health care – that’s our new vision. And we’re so excited to share this with you as we look towards the next three years, together.

This new plan puts you - our patients, our providers, and our community - at the centre, surrounded by the values we hold as an organisation.

A healthy community is a community where all individuals have access to comprehensive team-based primary care. Our Strategic Plan will guide us for the next three years - from 2025 to 2028 - and is aligned with Ontario's Primary Care Action Plan, which seeks to connect every Ontarian to team-based primary care by 2029.

We want to extend our sincere thanks to everyone involved in creating our new Strategic Plan – our Board of Directors, the Greater Hamilton Health Network (GHHN), our GHHN Primary Care Network partners, our staff, patients, family doctors, and healthcare providers. The new plan was a collective effort and we could not have done it without you.

We hope our new plan resonates, and we look forward to continuing to serve our local community. To view the full plan, including an accessible transcript, visit our website: https://buff.ly/Cup8SLi

May 5-11 is Mental Health Week in Canada! This year's theme, "Unmasking Mental Health," encourages us to have open and judgement-free conversations, seeing beyond the surface to the whole person.

At the Hamilton Family Health Team, we have an incredible team of mental health professionals - mental health counsellors, mental health nurses, psychiatrists, and social workers - who champion this approach and build their practice on a foundation of vulnerability and compassion. They use counselling, training, and a variety of other methods to assist individuals in our community with mental health, situation and social adversity. They listen to patient concerns and problems, develop a treatment plan, and aid patients with coping strategies.

Thank you to our mental health team - your dedication and expertise offer hope and guidance to the community and to your teams.
