Accessibility: It means different things to different people groups based on the type of functional disability. For wheelchair users, the most common modification is a ramp. But not just any ramp. BC has a building code for accessibility that requires all such modifications to be built accordingly. This means that the ramp must have a specific slope ratio (1:10 or 1:12), should have edges and hand rails. These are just the basics and there are other specifications. The code also deals with bathrooms, and entry ways. The code is a great place to start but if you have a specific client, there may be many things NOT in the code that you need to consider. So first, understand the person's abilities and disabilities if you are building for a specific person. Second, consider engaging an OT to help you. They can help you with the first item! Then ask the person who is going to be using the space. They have invaluable information on barriers as well as solutions. You may want to engage an architect with special training in accessibility as well.

Senior Solutions Part I: Did you know that the BC Government in 2016-2017 required all 24/7 residential care homes to provide manual basic wheelchairs for residents who are assessed by an OT/ PT as requiring one? These wheelchairs are also required to be serviced by the individual residential care home. That includes maintenance and repairs. However, many residents require higher level cushions and wheelchairs such as those that have a tilt in space feature. The cost of these items and any adaptation/ modification that is required must be covered by the individual or his or her family. Keeping this in mind, if you or your loved one have extended health benefits, these can be used to cover the cost of upgraded items or wheelchairs. If not, then the cost of such items can be extreme. Maintaining benefits can prove to be a wise investment for future needs.

How are occupational therapists able to help people recover from injuries? Occupational Therapy is a unique profession in that we consider all activities to be able to be understood only in the context of the individual (i.e. physical, mental, spiritual etc.) and the environment in which the task is performed. We carry out high level task analysis which allows us to hone in on the specific component of the task that is troublesome for the individual and analyze which aspects of the person and the environment contribute to the problem. In turn, this allows us to use our other specialized skill - task and/or environmental modification. We are proudly the only profession that provides this approach. Does pain cause functional limitations? This is a challenging question because pain is both physical and psychological. Pain can be acute, transient or chronic. And our approach to pain can be influenced by our mood and thought processes. For example, if an individual has a high level of perceived injustice, they are more likely to report higher levels of pain and disability. Similarly if an individual is experiencing anxiety or depression, he or she may also report elevated levels. How then can a Functional Capacity Evaluator determine what the individual's "true" level of function is? Should evaluators be swayed by displays of pain behaviours versus objective evidence of function? The answer is that function is the focus of evaluation and not pain. Pain must only be assessed in relation to it's impact on function.

A new way of looking at chronicity. The definition of chronic can be the duration for which something lasts. However, we also consider certain conditions as chronic meaning that they are not likely to change. However, it's a very fine line between those two definitions. There are many situations that have lasted longer than 6 months (the typical standard for referring to something as chronic) that can still be remedied. They may not be eliminated but they can be reduced or improved. But the definition of chronic can lead people to believe that nothing can change or be done for their situation. This is where Occupational Therapists can make a difference. Working alone or in conjunction with kinesiologists and psychologists, we can help people find ways to address their thought patterns and increase their participation in day to day activities.

Wondering what makes a great Functional Capacity Evaluation? Here's Five Must Haves:
1. A two day FCE for adults
2. Repeat tests - Start of day, end of day and Start of Day two that include affected and unaffected areas of the body
3. Distraction testing - tests that allow for testing without the evaluee knowing what exactly is being tested.
4. Work simulation component or embedded into the whole of the FCE.
5. Focus on functional performance that is reflective of the evaluee's actual daily activities which may mean changes to the above four.

ICBC's current list of approved services includes several passive therapies which can be helpful in the acute stages immediately after an injury. Passive therapies include physiotherapy, massage therapy, acupuncture and chiropractic treatment. But often we need help getting back into doing the tasks that we did before the accident. This is where an Occupational Therapist can come in handy. We can help you find ways to carry out the activities you did before. Just ask your physician or any treating professional along with your ICBC adjuster for a referral to an Occupational Therapist. We'd love to help you get back into life.
