Plum Tree Memory Care and the Centre for Family Medicine are pleased to announce the creation of a primary care Seniors’ Clinic. Our Seniors’ Clinic is a fullservice primary care clinic for senior Ontario residents, conveniently offered at our Kitchener clinic.

The clinic is nurse practitioner-led and is supported by other health care professionals such as family physicians, nurses, and pharmacists. Services are covered by OHIP. It specializes in providing care to older adults and is committed to providing top quality, comprehensive primary care.
