We're excited to share a new case study from Evidence2Practice Ontario (E2P) that highlights the real-world impact of our EMR-integrated tool for diabetes, which is bringing the best clinical evidence directly to the point of care.

In Ontario, an estimated 4.4 million people live with prediabetes or type 2 diabetes. One of the more serious and common complications associated with diabetes is diabetic foot ulcers, which cause about a third of all nontraumatic below-the-knee amputations in Canada. Yet only 53% of Canadians with diabetes reported receiving a foot exam by their healthcare provider in the past year.

Our study explores how one primary care clinician used the E2P EMR tool for diabetes management over a period of 7 months to improve patient screening, risk assessment, and patient education. The results from her practice speak for themselves:
- 82% of patients were screened for diabetes-related complications within the past 12- months.
- 80% of her diabetes patients received appropriate foot care, key to preventing and managing ulcers and lower-limb amputations.

By integrating quality standards directly into her EMR, this clinician streamlined care delivery and aligned her practice with the latest evidence, all while keeping the focus on patients.
