Scalp discomfort is a clinical sign.

Pruritus, burning, tightness, scaling, tenderness, or pain are indicators of scalp dysfunction, not cosmetic inconvenience.

Common contributing factors include:
• inflammatory pathways
• impaired scalp barrier function
• microbial imbalance
• hormonal, metabolic, or stress-related triggers

Symptom-based products may provide temporary relief, but resolution requires assessment—including scalp examination and, when indicated, internal testing.

Not all hair loss means the follicle is gone forever.

Many follicles simply go dormant — meaning they’ve stopped producing visible hair, but are still alive beneath the scalp.

Think of it like a seed in winter…
The right conditions can allow it to grow again.

A dormant follicle:
• Still exists in the scalp
• Has blood supply (even if reduced)
• Is in a prolonged resting (telogen) phase
• Is often producing very fine or invisible hairs

This is very different from a follicle that has been destroyed or replaced by scar tissue.

Empty follicles have potential. Empty follicles are typically dormant follicles that need to be “woken” up.
This, pictured here, we did in clinic. Yes, we need to do the work internally this takes time. In the mean time we also need to create a healthy eco system to sustain hair. These procedures are tedious and take time, but are definitely worth it.

Why scalp inflammation must be treated before regrowth.

Hair growth does not occur in an inflamed environment.

Chronic scalp inflammation alters the follicular micro-environment and interferes with the normal hair growth cycle. Even when follicles are still present, inflammation can suppress growth, prolong shedding, and accelerate miniaturization.

What inflammation does to the follicle:

• Disrupts the anagen (growth) phase
• Constricts micro-circulation around the follicle
• Impairs nutrient and oxygen delivery
• Triggers premature shedding (telogen shift)
• Contributes to follicular miniaturization
• In scarring conditions, can permanently damage follicles

Inflammation may be visible as redness, scaling, or tenderness—but in many cases it is only detectable under magnification.

Topical products, supplements, or stimulation therapies cannot override an inflamed scalp. When inflammation is present, the follicle prioritizes survival over growth. Attempting regrowth without first calming inflammation often leads to poor or temporary results.

Inflammation is a central feature in many forms of alopecia, including autoimmune and scarring types. Early identification and management are critical for follicle preservation.

Takeaway: Stabilization always precedes regrowth.

In my practice, scalp inflammation is assessed and addressed before initiating growth-focused protocols. This evidence-based approach improves outcomes and protects long-term follicle health.

However, there’s only so much a topical can do for you. Treating the scalp alone typically does not give you sustainability, meaning it can give temporary results….
Your first step to tackle hair loss is the proper assessment; this allows you to know what type or types of alopecia’s are present as well as if there is an under lying scalp disorder contributing to your hair loss. Every type of alopecia needs to be approached differently.
