Support that meets kids where they are đź’š

Assertive Community Treatment (ACT) is an intensive, community-based program for children and youth ages 8–17 with significant mental health needs who haven’t found success with traditional services.

Our multidisciplinary ACT team includes nursing, child and family therapists, occupational therapy, and recreational therapy. Together, they create individualized treatment plans that support each child’s unique needs.

The goal?
To help children and youth stay safe, supported, and thriving in their homes, schools, and communities.

ACT may be a good fit if your child:
• Is 8–17 and lives in London-Middlesex
• Has a mental health diagnosis or referral from a regulated health care professional
• Has high service needs or frequent hospital or crisis service use
• Has not responded well to traditional mental health programs
• Experiences moderate to severe challenges in daily functioning

ACT is not intended for youth primarily seeking support for:
• Addictions or substance use
• Developmental disabilities

Youth may not be eligible if they are too violent or have other significant risks that would impact safe community care.

Referrals are accepted through Tandem, including self-referrals, community agencies, hospitals, schools, and primary care providers.
