Retreat case with a lateral canal that created short working length reading and therefore instrumentation during initial treatment. There was a ledge feel on retreat due to that reading. A tricky case without CBCT. In this case the region apical to the lateral canal was filled with sealer that flowed from the buccal into the palatal canal.

I treated this tooth 10 years ago and thought I did a good job, having treated 4 canals. Clearly, I missed some anatomy towards the lingual of the distal root. I retreated the distal root 5 years ago. The tooth looks healed.

The CBCT image shows intricacy of the distal root anatomy, with 2 more canals. It also illustrates how my instrumentation philosophy has changed toward smaller shapes.
Still learning.

1 year recall on conservatively instrumented lower molar. Looks like things are healing adequately. Always good to see a nice outcome. A bit more healing to go.
