Extended Families Support Services Extended Families provides support for individuals and families in their homes and community.

SUPPORT SERVICES:

-1:1 Personal Care and Companionship
-Homemaking Services
-1:1 Respite Support in home up to 24 hour care
-Medication Reminders
-Assistance with Physiotherapy Exercises
-Meal Preparation and Grocery Shopping
-Support with medical appointments including advocacy, assistance with communication, tracking, and follow ups
-Property Maintenance & Gardening Services
-Live in Caregiving Options

FAMILY CONSULTANT SERVICES:

-Service coordination and advocacy
-Ongoing assessment and support towards emotional, psychological, and physical safety and well- being
-Safety check ins including assessment -Caregiver Support and respite care
-Provide long term continuity of care
-Advocacy for support and services
-Referrals and extensive screening processes for safe and trustworthy service providers
