Southwest Ontario Aboriginal Health Access Centre Providing wholistic health/community services to Aboriginal people in Southwest Ontario; Traditional

Aboriginal Health Access Centres (AHACs) are innovative, Aboriginal specific, Indigenous informed health care agencies. They provide a combination of health and social services to First Nations, Métis and Inuit communities, including:

Traditional Healing
Primary Health Care
Health promotion services
Cultural programs
Community development initiatives
Social support services

In Ontario, AHACs were first announced in 1995 in response to epidemic, systemic health disparities and inequities within the Aboriginal population across Ontario. They operate from a wholistic Aboriginal health framework which is often referred to by Aboriginal people as “the good life” or as restoring the “good mind”. The framework focuses on the restoration and rebalancing of the physical, mental, emotional and spiritual wellbeing of Aboriginal people, families, communities and Nations, and understands “culture as treatment”.

We're hiring: SOAHAC is currently seeking a Diabetes Nurse Educator to provide culturally safe, client-centered diabetes education and support to Indigenous adults living with Type 2 diabetes and pre-diabetes at our London site.

Working as part of a multidisciplinary Diabetes Education Program team, the Nurse Educator supports clients in developing practical self-management skills, improving health outcomes, and accessing coordinated care.

The role includes individual counselling, group education, outreach activities, and participation in program monitoring and evaluation in alignment with Ministry-funded Diabetes Education Program requirements.

$79,672 – $99,590 annual salary
35-hour work week
HOOPP pension + comprehensive benefits
Generous paid time off, including cultural and spiritual days
