Assisting a loved one to return back home after a hospital stay can present many challenges. At Helping Hands, we aim to support those stresses with our Transitional Care program.

This program is designed to offer 72 hours of comprehensive care services tailored to facilitate a safe, supported and sustainable transition back home. The selected services can be customized to include a nurse and/or a Personal Support Worker (PSW), offering round-the-clock care and/or a customizable hourly approach to accommodate the individual needs of both the client and their family. This is an example of how we are bridging care to ensure that our clients receive the services they need when they need them.

When it comes to our long-term care and retirement home clients, it's about those in our care at the end of the day. That's why we recently visited Copernicus Lodge in Toronto to deliver residents some flowers in hopes of brightening their day. Their smiles are what matters to us!
