I work as an orthopaedic and Pelvic Health Registered Physiotherapist. I graduated from Western University’s School of Physical Therapy with a PhD and Masters of Physical Therapy in 2021 and 2020, respectively. I also hold a Master’s of Science degree in Behavioural and Cognitive Neuroscience and an honours Bachelor degree in Psychology.

I have post-graduate training in ‘Technologies to Support Pelvic Health’, ‘Labour and Delivery: Maternal Support through Comfort Measures and Pelvic Biomechanics’, ‘Caesarean Birth’, ‘Pelvic Girdle Pain’, ‘The Physical Therapy Approach to Male and Female Urinary Incontinence’, ‘The Physical Therapy Approach to Male and Female Pelvic Pain’, and Mens Pelvic Health including Recovery from Prostatectomy and Peyronie’s disease.

Meet Dr. Guitar! I did it. I managed to drag myself across the finish line during a global pandemic - and I am damn proud!

In clinic, I will still be going as Nicole - because in Canada physiotherapists with doctoral credentials don’t use that title when assessing or treating patients (this is because the highest - clinical - level for physiotherapists in Canada is Masters level!). However; outside of patient care find me using that title on the regular!

A PESKY little muscle called puborectalis slings around the re**um to generate a kink that helps to prevent unwanted accidents when we’re standing. This muscle is the MOST relaxed in a supported squat (knees above hips!).

Tips for the perfect p**p:
💩Elevate knees above hips.
💩Do not perch on the edge of the toilet with your toes on the ground.
💩Keep knees and feet apart.
💩Lean forward and rest your elbows on your thighs.
💩Relax. Breathe
💩Do not hold your breath when trying to move your bowels.

Your bladder is a muscle!

It’s also called the “detruser” and can normally carry 10-20 ounces of fluid. The bladder is a storage area and, like other muscles, it can be trained!

When the walls of your bladder stretch to hold more fluid, receptors in the muscle are stimulated, travel to your brain and make you aware of your need to void! This causes muscles of your pelvic floor to to contract and prevent urination.

To empty your bladder, it must contract, and the muscles of your pelvic floor must relax! This means that urination should feel like a passive event (you’re not pushing to empty: like you might do when you’re in a rush or squatting over a toilet!).

Normally after voiding, 1-2oz of fluid remains in your bladder (this is why you might feel like you can still push a bit of urine out when you’re done!).

Pelvic health physiotherapists can assess so much more than how the pelvic floor may be affected by tension or pain, weakness, or asymmetry.

The pelvic floor is influenced by local and distant muscle actions. Our assessment of your pelvic floor includes a comprehensive screen that includes questions about you and YOUR story. When you come for an assessment with a pelvic floor physio we’ll start by just talking. This is an important part of understanding your goals and what matters most to you.

Next, we do a screen of your muscle function (external and internal muscles, if appropriate). This includes assessing your breathing pattern, your posture and movement, range of motion, gait and strength.

We can gain a lot of understanding about what is happening in your pelvis from observation of these things, but we are also rostered to perform internal assessments of both the va**na and re**um. Performing an internal exam is a controlled act in Canada and pelvic floor physiotherapists are trained to perform these exams with your permission and if YOU are comfortable. 

From here, we can discuss with you all the treatment options that might work for you. This might include any combination of: education, pelvic floor muscle training, tracking (e.g., bladder diaries), bladder training, UTI prevention, breathing and mediation training, prolapse specific treatments, electrical stimulation, biofeedback, and manual therapy.

A healthy pelvic floor is so much more than a strong pelvic floor.

You often hear the misconception that pelvic floor problems (like leakage or pain) are caused by weak pelvic floor musculature. MOST of the time, this isn’t true at all. In the last year I’ve treated maybe 2 people whose true primary issue was in fact, muscle weakness.

A healthy pelvic floor is one that is coordinated, able to relax and contract through a full range of motion, support healthy function of the bowel and bladder, and sexual function. It usually is accompanied by a regulated stress and immune response and a healthy gut.

Stop thinking that if you have problems you must be weak and that simply doing kegels will be the answer to your pelvic floor dysfunction.

I recently tried product for period pain and I REALLY liked it! For context, I have painful periods, with a suspected endometriosis diagnosis 🎗

The Mira pro is a TENS machine - also known as a Transcutaneous Electrical Nerve Stimulation device (like a Dr. Ho). It applies a small electrical current through two pads placed directly on the skin that activate sensory nerve roots.

This is effective for pain relief because of the Gate Control Theory of Pain! Have you ever banged your funny bone of off something? Was the first thing you felt inclined to do rub it? This habit is using gate control to activate different nerves in the area to override pain messages! It can prevent or reduce pain messages from travelling to the brain 🧠

In the pelvis, TENS can be used abdominally or on the low back for pain (I did both!). It can’t be used if you’re pregnant, have local metal or electronic implants, or over irritated skin.

The best part about this device was that it didn’t have any wires and could be discreetly worn under pants (I actually worked a whole day with it on me, turning it on and off or changing settings as needed). It charges just like your phone and then is good to go for a whole day. It definitely doesn’t eliminate pain, but I did find it helped me get through a painful day without relying on pharmaceuticals 💊!

An episiotomy is a surgical cut in the perineal muscles (the area between the va**nal opening and the a**s) to enlarge the va**nal opening during birth.

In 2009, The Canadian Maternal Experiences Survey indicated that 20.7% of birthing persons who delivered va**nally (or attempted to) experienced an episiotomy.

Evidence shows us that there is no beneficial maternal or fetal outcomes associated with the use of routine episiotomy (Muraca et a., 2018; Viswanathan et al, 2005). Tearing that occurs along these muscles “naturally” heals better than muscle tissue when surgically cut.

Ask your health care providers about their routine use of episiotomy and learn more about when it would be medically necessary to have one!
