Chronic low grade systemic inflammation should be considered a key subgroup of musculoskeletal pain.

Treatment is more complicated than simple "lifestyle" interventions.

Causation must be addressed.

One cause is ongoing infections or sub-clinical infections. Of these, one of the most common is related to gum disease.

Is Joint Hypermobility Syndrome a unique subgroup of musculoskeletal pain?

Table 1. Classification of joint hypermobility*
1. Can you now (or could you ever) place your hands flat on the
floor without bending your knees?
2. Can you now (or could you ever) bend your thumb to touch your
forearm?
3. As a child did you amuse your friends by contorting your body
into strange shapes OR could you do the splits?
4. As a child or teenager did your shoulder or kneecap dislocate on
more than one occasion?
5. Do you consider yourself double-jointed?
* Affirmative responses to 2 or more questions were used to classify
subjects as having joint hypermobility.

Positive responses to 2 of these 5 questions had a sensitivity of 84% and specificity of 85% in 2 cohorts of 382 patients

An unknown mechanism of knee valgus is a "soft neurological pattern".

As weight is changed through the foot, the pressure changes of the stimulus region of Babinski and Foot Tendon Guard can cause a movement and tone change.

Adult-Minor Neurological Dysfunction should always be considered as a cause of altered movement patterns.

Primitive reflexes should be checked (postural reflexes as well).

Neurological soft signs are very common in MSK pain populations.

If the pt can cognitively override the primitive reflexes, the reflexes may not need Rx. However, Rx can make the rehab & motor skill learning process better.

NOTE: Most the research done on MSK pain has not considered the immune system.

The absence of consideration of the immune system voids the relevance of a substantial body of our evidence base.

It represents a neglected subgroup and must be considered a key mechanism and cause in MSK pain.

Currently researching a questionnaire as a surrogate measure for chronic low grade systemic inflammation.
