On behalf of all staff at Active Health Chiropractic, we wanted to inform you of recent changes to our Mandatory Covid screening questionnaire.
We have added this question:
***Have you or anyone in your household been directed to self isolate by Public Health or have you been around anyone who is self isolating?***
If the answer is “Yes” then you will be required to call and cancel your appointment. Please note, the rescheduling of these appointments may vary due to Public Health recommendations. This policy is in effect immediately due to the confirmation of community spread.
Please also take the time to thoroughly read our Survey and have it COMPLETED prior to leaving for your appointment. Patients that do not have their questionnaire filled out may be denied treatment.
We continue to follow our Provincially regulated screening, cleaning and disinfection protocols with every patient, every day to protect our staff and especially our patients.
We are so very grateful for our wonderful patients and want to thank you for your continued support as we get through this together!
