Most injectors don’t offer under-eye filler, and there’s a reason for that. The tear trough is a delicate area with multiple arteries and very thin skin, so the margin for error is small. Product choice matters a lot. I use Redensity II, which was designed specifically for the under-eye area and tends to attract less water than many other fillers. If a filler pulls too much water into this region, it can create puffiness, which is the opposite of what we’re trying to correct. Because of its unique formulation, Redensity II is also my preferred filler for treating perioral rhytids (smoker’s lines) around the mouth. Under-eye filler is NOT for everyone. LIP PROGRESSION | Why slow is better when it comes to lip filler. This first-time mom felt like her lips were disappearing and wanted a little hydration back in the tissue. Rather than rushing to a full syringe in one appointment, we built her lips gradually over two visits, a half syringe (0.6 ml) at a time. Splitting up the full syringe into two visits allowed us to first establish the vermilion border, setting the stage for more volume with a reduced the risk of filler migration. THE MYTH YOU MIGHT STILL BELIEVE ABOUT BOTOX | Botox isn’t a facelift and it doesn’t lift the SMAS. But that doesn’t mean it has no role in the lower face or neck. Neuromodulators work on muscle, not deep facial structure. When used thoughtfully, they can soften downward pull, reduce neck banding, and help refine the jawline by relaxing overactive muscles. What they can’t do is lift descended tissue the way surgery can. RIGHT TOOL, RIGHT PLACE | Nope, you can't split your full syringe across your lips and jawline. The formulas I choose for each treatment area are highly specific. Different areas of the face require different levels of structure, flexibility, and support. Revanesse® Sculpt™ is a dermal filler designed for areas that need stronger structural support. It’s typically placed in deeper planes of the face to restore volume and create definition, most commonly in the cheeks, chin, and jawline. This is very different from the softer fillers used in more delicate areas like the lips or fine lines, where flexibility and movement are more important. 360 APPROACH | Life is 360°, so your results should be too. During your consultation, you’ll see me moving around your face to assess it from every angle. I’m looking at the profile, the three-quarter view, how the face sits in motion, and how everything balances together — just as the rest of the world will see your results, from every angle and through every expression. Now, I don’t always remember to take before and after photos from every angle… but that’s a whole other conversation.

BOTOX: THE REAL TIMELINE | One way I often explain neuromodulator longevity to patients is by thinking about two different bell curves. The first bell curve is biologic duration. Some people fall on the shorter end of the spectrum and may notice their treatment wearing off closer to two months. Most people fall into the average range of about three to four months. And then there are patients who metabolize it more slowly and may see results lasting five months or longer. But there is a second bell curve that matters just as much, and that is patient preference.

Some patients prefer absolutely no movement and will return earlier in the tapering phase of the treatment. Others are perfectly comfortable allowing some movement to return before they come back. Where you fall on that curve often determines how frequently you choose to treat. It’s also important to understand that movement returning does not always mean wrinkles returning. When skin is healthy and well supported, many patients tolerate some muscle movement without being bothered by it. Cosmetic injections do have limitations, and there are always nuances involved. Dose matters. The specific neuromodulator used can influence how gradually the treatment tapers. And individual biology plays a role in how your body responds over time.

This is why the relationship between patient and injector is so important. A thoughtful treatment plan develops over time as your provider learns how your face responds, how you prefer to look, and where you feel most comfortable in that tapering phase. When patients move from injector to injector, it becomes much harder to refine that plan. It takes time to truly understand someone’s facial anatomy, muscle patterns, and treatment preferences.

THE LIP INTRO | Here’s why I often start with a half syringe of lip filler:
1) It allows me to see how your body responds to the product. Everyone metabolizes hyaluronic acid fillers a little differently.
2) It allows us to build slowly and maintain control over shape and proportion. Gradual layering also helps reduce the risk of filler migration, when product moves beyond the vermilion border and into surrounding tissue.
That said, many patients who start with a half syringe end up returning for the second half sooner than they expected because they miss the swell 😂
