29-Year Success Story: Lower Porcelain Veneers Still Going Strong! 🦷✨
These updated photos were captured this summer during a routine hygiene visit - and these Empress veneers were placed way back in 1996!
When I bonded these nearly three decades ago, I never imagined they’d still look this incredible. The margins remain really good, and honestly? I’m wondering if we’ll hit the 35-year mark or beyond.
The secret to longevity: Conservative preparation preserving plenty of enamel for optimal bonding + precise fit and occlusion design= exceptional long-term results
This case continues to remind me why meticulous technique matters so much in our profession.
Question for my fellow dentists: What’s the longest you’ve had porcelain veneers last in your practice? Drop your record in the comments! 👇

🦷 Final Lower Restorations – Full-Mouth Rehab Case Update 🦷

Following up on the provisional phase, I’m now sharing the final lower restorations for this full-mouth rehabilitation case. These restorations include a combination of layered e.max veneers and onlay veneers, and monolithic molar zirconia crowns beautifully crafted by master ceramist Harald Heindl to deliver both esthetics and durability.
📸 You’ll also see photos of the finished lower with upper provisionals, along with the lower preparations. The provisionals played a key role in refining both the occlusion and smile design prior to final ceramics.
🔜 Next up: completing the final upper restorations—stay tuned for the full case outcome.

7-Day Post-Op Results – Upper Full-Arch Rehabilitation

Here are the 7-day post-op photos following an upper full-arch rehabilitation case.
Treatment included:
* Laser gingivectomy
* Core build-ups for structurally compromised teeth
* 14 layered zirconia crowns to open the vertical and restore worn, eroded teeth
*
The ceramics were crafted by master ceramist Harald Heindl. Each restoration was layered on the facial for esthetics and kept monolithic at the occlusal contacts for strength and longevity.
This patient has large masseter muscles and a very high bite force, which directly influenced both the restorative design and material selection.

We also lengthened the upper teeth to enhance tooth display, whitened the lower arch, and completed targeted injection molding using Filtek Supreme on five lower teeth to address wear and erosion. These steps improved both esthetics and occlusal stability.

Full-Mouth Rehabilitation: Provisional Restorations
Here’s a look at the provisional restorations from a recent Full-Mouth Rehabilitation case I prepared.
🔹 Clinical Challenges:
This patient presented with severe erosion, attrition, a deep overbite, and significant loss of vertical dimension, with the lower incisors worn almost to the gum line.
🔹 Treatment Plan:
✔️ I referred the patient to Dr. Naheed Mohamed, a periodontist, for crown lengthening to allow us to preserve the natural teeth without the need for endodontics or dental implants.
✔️ A diagnostic wax-up was created by master ceramist Harald Heindl, which guided the new vertical dimension for both the direct composite resin onlays and the provisional restorations.
✔️ Opened the vertical dimension using direct composite resin onlays on the upper molars and one bicuspid.
✔️ Provisional restorations were placed in harmony with the new vertical dimension, allowing us to evaluate function and esthetics before final restorations.
💡 Educational Insight for Dentists:
This case was featured in the December 2024 Occlusion Design webinar, where I walked members through the step-by-step fabrication of direct composite resin onlays with clinical videos to enhance the learning experience. Cases like this are exactly what I share in our monthly Occlusion Design webinars, providing detailed insights into advanced restorative techniques.

🦷 Provisional Restorations in Full-Mouth Rehab 🦷
Some Case Details for my Dentist Friends: This full-mouth rehabilitation case involved opening the vertical dimension and enhancing tooth display to improve both function and esthetics.

🔹 Treatment Plan:
✔️ Combination of veneer, onlay veneer, and crown preparations
✔️ Designed to re-establish anterior guidance for long-term occlusal stability
✔️ Custom provisional restorations placed immediately after preparation

⏳ Key Learning Point: Provisional restorations are essential for evaluating esthetics, function, and patient adaptation before finalizing the case. Over the next week, the gingival tissue will stabilize, improving overall integration.

📸 I’ll be sharing the final restorations soon! What’s your biggest challenge when working with provisionals in full-mouth cases? Let’s discuss in the comments! ⬇️
