📌 ATTENTION: If you have a benefit card that looks like this - with Carrier 12 and Group 055555

🎯 As of July 1, 2023 you MUST have your NEW Canada Life card with you upon presenting a prescription or asking for a refill. We need to know the correct Group Number on your new card as it will change.

If you do not have this new information with you when you ask for a prescription it will result in a delay. We have no way of knowing what your Group number will be, that will be your responsibility to find out and present to us.

If you need help finding out your information from July 1, 2023 onwards you can call Canada Life at 1-855-415-4414.

We cannot call on your behalf.

There will be many changes as of July 1, 2023 so it will help to be prepared.

Starting on October 15, Ontarians can download an enhanced COVID-19 vaccine certificate with a unique QR code. Using the QR code makes it easier to verify your vaccination status in settings requiring proof of vaccination.

The QR code vaccine certificate will make it easier and more convenient for patrons. Valid personal identification will still be required to confirm identity. ontario.ca/get-proof

In response to the Province’s announcement to accelerate the rollout of its vaccination plan, we are moving forward with a vaccination schedule for the month of May, contingent on supply.

We continue to follow direction from the Province on prioritization and our plan is based on age & risk. Halton is making essential workers a priority as there continues to be higher rates of infection among those who cannot work from home. Age also continues to be a predictor for a higher risk of severe illness and death.

We are continuing to vaccinate priority groups identified by the provincial government. Starting March 19, more eligible Halton residents can book through our online system, including residents aged 75+ and First Nation, Métis & Inuit adults who live in Halton aged 55+.

Eligible residents can still book appointments using Halton’s easy-to-use online booking system. If you visit the Provincial booking system, you will be redirected back to our system. Clinics are now open in all municipalities ( , and ) but please do not call clinic locations directly and only call 311 if you require assistance—this will ensure those who need support have access to a customer service representative.
