Family Health Teams are health care organizations that:

Include a team of health care professionals (i.e. physicians, nurse practitioners, nurses, pharmacists, social workers, dietitians and others) who work together to provide health care for the community;
Ensure that people receive care they need in their communities by providing better care, closer to home;
Focus on chronic disease management, disease prevention and health promotion and work with other organizations in the local community such as Public Health Units and Community Care Access Centres.

The Owen Sound Family Health Team, Georgian Bay Practice Associates, and the Owen Sound Medical Associates will be closed Monday September 1st for Labour Day.
The Owen Sound Family Health Team will also be closed Friday Aug 29th. Physician offices will be open.
