Did you know …

Older adults who were discharged from hospital into a home-care setting were 40 per cent more likely to be readmitted to hospital within 30 days of discharge, according to University of Alberta research.

At Comfort Keepers Peterborough we have a high success rate of preventing readmittance.

How do we do this?
-we meet with you, the client, hospital staff and come up with a hospital to home plan

-train our staff on the specific care needed once home and Importance of communication

-constant communication between our care staff and family members on the progress

-reassess the clients needs once home on a weekly & monthly basis

Comfort Keepers provides a wide range of in-home senior care services for the surrounding areas. In-home elder care provides family members with peace of mind, knowing their loved ones are taken care of. Providing long-term care for an elder can be very taxing and time consuming. Hiring home support services such as Comfort Keepers reduces this stress.

Through our unique system of in-home health care we call Interactive Caregiving we help our senior care clients enhance their independence and sense of well-being. The three things that can lead to seniors losing their independence are: falls that lead to hospitalization, medication mis-management, and under or mal-nutrition. Senior safety is the cornerstone of our approach to elder care and Interactive Caregiving because insuring in-home safety directly affects the well-being of our clients. We also directly address elder nutrition through our STOP Hunger community awareness campaign.
