I truly believe physiotherapists should be able to refer for medical imaging when appropriate for musculoskeletal injuries. This would require further training and certification, which we welcome and look forward to.

Adding this to our scope of practice will improve physiotherapy quality of care and also reduce some of the burden on other sectors of the healthcare system.

Updated clinical practice guidelines for the treatment of hip osteoarthritis list manual therapy, IMS dry needling, and exercise prescription among the top for physiotherapy interventions. This is consistent with Riverwood's belief in a multimodal approach to many different conditions that we see here in the clinic.

Important exercise parameters for achilles tendinopathy included: intensity of contraction, total time under tension, number of sets and reps, type of contraction, and avoiding excessive stretch on the achilles early on for insertional tendinopathies.

Intensities of 70-90% of max voluntary contraction were most effective at creating a physiologically positive change in the tendon. In other words, some appreciable weight is required during rehab exercises.

More intense exercise should be reserved for later stages of rehab when the tendon is less acutely painful and weak.

You are all now achilles tendon experts!

Another club volleyball season has started up and with it comes another version of Riverwood's Workout to Warm-up that we design for these athletes.

The program blends some classic mobility and strength exercises with a few volleyball specific drills. All teams from U12 to U18 participate in some version of this and are encouraged to run through the exercises before every practice and tournament.

We want athletes to be physically and mentally prepared to play, and as they continue to consistently work through this program year after year, they will move better and better.

We had a blast but, aside from the closest to the button contest, the Riverwood crew definitely could use some more curling lessons. Thank you to Judy and Kevin from the Port Moody Curling Club for teaching us the basics.

Try telling a barbell athlete not to use a barbell 😂. I really appreciated the emphasis this course put on making smaller changes to form and programming in the presence of injury, BEFORE larger modifications to the movement or removing it completely.

Sometimes we can modify programming to allow for some relative rest throughout the week while still being able to workout using the same exercises. Other times, small changes in positioning could help (eg. Try a wider stance or more toe out during a back squat if you are getting anterior hip pain at depth).

It's not always that simple but, wherever possible, we try to keep athletes doing what they love in one form or another while they rehab.

Simple to do, and they hit on a lot of key areas. I like to tac these on to the start/end of a workout or even as an early part of a pre game warmup. Most importantly though, just find some regular, consistent times to get work like this done so your body continues to move well and stays game fit!

Mobilizations with movement are manual therapy techniques that have a solid amount of positive research behind them.

In keeping with treatment trends, treatment in the clinic is great, but works best in combination with a consistent home exercise program.

These are three of my favorite home exercise versions of mobilizations with movement for the neck. I prefer to use a heavy band to apply the mobilization but it could be something like a towel as well. The pressure applied should be in a location and direction that significantly improves pain levels and allows for greater range. If it doesn't, it is not worth pushing through.

If you have mechanical neck pain and stiffness with certain movements, these may be worth a try!
