A new study published in the American Journal of Obstetrics and Gynecology looked at whether vaginal estrogen therapy is safe for women who have had breast cancer.

Vaginal estrogen is a treatment used to help with symptoms of genitourinary syndrome of menopause (GSM) like vaginal dryness, pain during s*x, or urinary issues, which are common after breast cancer treatment. Here’s what the study found:

Vaginal Estrogen is Safe for Most Breast Cancer Survivors:
The study combined data from 24,060 women across 6 studies and showed that vaginal estrogen doesn’t raise the chance of breast cancer returning. It might even slightly lower the risk

No Higher Risk of Death:
The study also looked at whether vaginal estrogen affects the chances of dying from breast cancer or from any cause. It found no increased risk of death, meaning vaginal estrogen appears safe in terms of survival.

Helps Quality of Life:
Vaginal estrogen can make a big difference for women dealing with uncomfortable symptoms like dryness, incontinence or pain. It helps them feel better and improves their day-to-day life without adding major risks.

Why This Matters: Many women avoid vaginal estrogen because they’re worried it could bring back their cancer. This study suggests that for most breast cancer survivors, it’s a safe option to relieve symptoms

The study noted that women on aromatase inhibitors (AIs), a breast cancer treatment, may need extra care, always talk to your doctor to find the best option for you

Know Your Breasts: Breast Self-Exam Guide 💪

Early detection saves lives! Do a monthly breast self-exam (BSE) to spot changes. Based on BC Cancer guidelines:

📅 When: 3–5 days after your period (or same day monthly if post-menopause)

👀 Step 1: Look in the Mirror
• Stand with arms relaxed, then raised, then hands on hips
• Look for changes in size, shape, or color
• Watch for:
• Skin that’s wrinkled, dimpled, or like an orange peel
• New bulges or dents
• Ni**le turning inward (if normally outward)
• Ni**le discharge
• Size/shape changes

👐 Step 2: Feel (Standing/Sitting)
• Use finger pads, moving in small circles with light, medium, firm pressure
• Check entire breast: collarbone to abdomen, armpit to cleavage
• Feel for lumps, thickening, or pain

🛌 Step 3: Feel (Lying Down)
• Lie with a pillow under your shoulder
• Use opposite hand to check each breast/ni**le area
• Use light, medium, firm pressure to feel all tissue layers

💡 What to Look For
• Lump/thick area different from rest of breast
• New size/shape change
• Dimpled, puckered, or orange-peel skin
• Ni**le discharge or pain

🚨 Found Something?
• Don’t panic—breasts can feel lumpy naturally. Key is noticing new differences. See a doctor if you notice something has changed

💡 What’s Normal?
• Breasts have lumpy/bumpy texture from glandular tissue
• Normal lumps feel smooth/rubbery, move slightly, similar in both breasts
• Some lumps are more noticeable pre-period due to hormones—often tender, resolve post-cycle

⚠️ What’s Concerning?
• Cancerous lumps feel hard/firm (like a stone/pea), fixed, doesn’t move, distinct from surrounding tissue, and is often painless (though some feel pain)

🌸 When to See a Doctor
• A new lump persisting post-period
• A lump different from rest of breast/other breast
• Skin changes (dimpling, puckering, redness, rash)
• Ni**le changes/discharge

Let’s talk about genetic testing

About 5–10% of breast cancers are hereditary. The BC Cancer Hereditary Cancer Program (HCP) offers free genetic testing and counseling in British Columbia to identify mutations (e.g., BRCA1, BRCA2, PALB2) that increase breast cancer risk. Here’s who should test and when.

What is the HCP (Hereditary Cancer Program)?
A provincially funded program providing genetic counseling and testing for hereditary cancer risk. They test blood or saliva for mutations linked to breast, ovarian, and other cancers. They also offer post-test support and referrals to high-risk screening clinics.

Who Should Test?
High-Risk Individuals such as those with:
🔸Personal history of breast cancer before age 50 or triple-negative breast cancer.
🔸Ovarian, fallopian tube, or peritoneal cancer (non-mucinous) at any age.
🔸Male breast cancer or multiple cancers (e.g., breast and ovarian) in the same person.
🔸Family history of breast cancer (before 50), ovarian, pancreatic, or high-risk prostate cancer (Gleason ≥7) in first-degree (parent, sibling, child) or second-degree relatives.
🔸Known BRCA1/BRCA2 or other cancer gene mutation in a family member.
🔸Ashkenazi Jewish heritage (1 in 40 carry BRCA mutations).

Most people without these factors don’t need testing, but you can discuss family history with your doctor to determine what course of action is best for you

⏰When to Test?
Start by age 25–30 for risk assessment, especially if your family history suggests hereditary cancer. Test the family member with cancer first (if possible) to identify mutations. You may self-refer to HCP if a relative shares their mutation details (e.g., report, family reference number).

❓Why It Matters
✅If a mutation is found it enables you to discuss risk-reducing steps with your doctor such as enhanced screening (MRI, mammograms), preventive surgery (e.g., mastectomy reduces risk by 90%), or medications.
✅If results are clear, this provide peace of mind

Why Routine Mammograms Aren’t Recommended for Women Under 50 in BC

In BC, routine mammograms are optional for women 40–49 and recommended every 2 years for women 50–74. Here’s why:
1. Lower Breast Cancer Risk in Younger Women
Only around 17% of breast cancers occur in women 40–49, compared to around 50% in women 50–74. Screening detects 5–7 cancers per 1,000 women 50–74 vs. 2–3 in 40–49 year olds.

2. Higher False Negatives in Younger Women
25–30% of mammograms in 40–49-year-olds miss cancers due to denser breasts. For women 50–74, false negatives drop to 10–15% as breast tissue density decreases.

3. Higher False Positives in Younger Women
9–11% of screens in women 40–49 are false positives, vs. 6–8% in women 50–74. This can lead to unnecessary stress, tests, and biopsies.

4. Mortality Benefit is Higher in Older Ages
Screening 40–49 reduces breast cancer deaths by 15–25% but prevents only around 1–2 deaths per 1,000 screened. For 50–74, 3–4 deaths per 1,000 are prevented, making routine screening more worthwhile.

5. Radiation Risk
Each mammogram delivers around 0.7 mSv of radiation (about 3 months of background exposure). Cumulative risk over decades is minimal but more relevant in younger women at lower risk.

6. Dense Breasts & Risk Factors
Dense breasts are more common under 50, making mammograms less accurate. Women with risk factors (e.g., family history, BRCA mutations) may need earlier or more frequent screening through BC’s Hereditary Cancer Program.

7. Shared Decision-Making
Women 40–49 can self-refer for free mammograms every 2 years but should talk to their doctor to weigh the risks and benefits.

If you have symptoms (lumps, discharge, skin changes), seek diagnostic imaging (MSP-covered with referral) regardless of age

Let’s talk about Mammogram Guidelines in British Columbia. 1 in 8 women in BC will face breast cancer. Early detection saves lives!

The BC Cancer Breast Screening Program offers free screening mammograms to individuals aged 40 and older without breast cancer symptoms

The Guidelines
✅Ages 50–74: Regular mammograms every 2 years are recommended. If you are considered high-risk, annual screening is advised
💛Ages 40-49 and 75+: Routine mammogram screening is optional, unless you are considered high-risk, in which case you are advised to screen annually. Stay tuned for our next post weighing the benefits against the risks for early screening.
❗️Who is Considered “High-Risk”: those with BRCA1/BRCA2 or other gene mutations, a first-degree relative with breast cancer, a personal history of breast cancer or DCIS, high-risk breast conditions like ADH, ALH, or LCIS, chest radiation therapy before age 30 (e.g., for lymphoma), a 20–25% or higher lifetime breast cancer risk based on risk assessment tools, or hereditary cancer syndromes such as Li-Fraumeni or Cowden. These individuals require annual mammograms, possibly with MRI or ultrasound and they may also be referred to BC Cancer’s Hereditary Cancer Program for genetic risk assessment.

📆How to Book a Mammogram: No doctor’s referral is needed for screening mammograms. Contact a BC Cancer Screening Program clinic or visit www.bccancer.bc.ca to find a location, including mobile mammography services.

👀If you notice symptoms like lumps, ni**le discharge, or skin changes, this requires investigation regardless of your age. If you have symptoms, don’t go to a screening center. Instead, your doctor will order a diagnostic mammogram and/or ultrasound
