Prescott Family Health Team (PFHT) is a group of health care professionals working collaboratively to provide comprehensive and accessible health care.

Services are provided by a intradisciplinary team including administration, reception, dietitian, nurses, nurse practitioner, physicians, and a social worker, Nurse Practitioner and Physician Assistant. Patients and families have access to an internist and psychiatrist; these services are fully integrated at the Prescott clinic.

🧑‍🎄🎅That is a wrap for our 2025 Christmas season!🧑‍🎄🎅

🎄We celebrated our annual Christmas Sweater Challenge yesterday in the office, which ended with a festive potluck luncheon. We truly have the most creative bunch!🎄

Congratulations to Lynn, whose Christmas Cheer Sweater took first place this year.

Amidst all the Christmas festivities today, we took a very special moment to celebrate Dinny's birthday 🧁💃🎁🎈Even though it is not until the 22nd, we had our whole team together to celebrate.

We wish Dinny the happiest of birthdays and hope she had a great day full of laughs, food, and wonderful wishes.

The competition was fierce once again at Prescott Family Health Team🎄 This year marked our 4th Annual Door Decorating Challenge.

A very special thank you goes out to our 4 wonderful judges, Susan Leonard from Walker House Adult Recreation Centre, Tracy Bellinger from Prescott Fire Department, Jody Botham from Grenville OPP Detachment and Anne Gillard from Prescott Public Library. We appreciate you all for taking the time out of your busy day and help join in the fun!

Saturday night we kicked off our Christmas season with our Annual Christmas Party 🥳🎄

We ate, laughed, played a couple games and enjoyed each others company!!

****This is the final week to gather gifts for BBBSLG and support our local children this holiday season****

We had some special visitors on Friday from Big Brothers Big Sisters of Leeds and Grenville. Special thank you to BBBSLG for bringing by some gift tags for our Giving Tree Campaign. This is our third year and Prescott Family Health Team is excited to give back to our community and their families.
Here's how it works;
Come into the office at 555 King St W Prescott to pick a gift tag from our tree, purchase a gift and return it unwrapped to our office with the tag attached.
BBBSLG will pickup all the donations from our office on December 15th.
**You do not need to be a patient to come in to get a tag, please the more the merrier!!
So please choose a tag, spread the joy and make a difference in our community 🎁🧸🎅☃️

Prescott Family Health Team was honored to present at AFHTO 2025, showcasing our innovative approach to organizational well-being and team culture in primary care. Our Social Worker and Counselor, Tim Mack, and our Executive Director, Dr. Dinny Mathew, co-presented Cultivating Thriving Teams: A Longitudinal Systems Approach to Organizational Well-being in Healthcare.

Faced with staff shortages, burnout, rising expectations, and funding constraints, we knew that meaningful change had to begin with our people. Therefore, we built a systems-based, employee-led wellness strategy that reshaped how we work, connect, and care for one another.

Our Results Have Been Transformative!

From baseline to 2025:
✨ Staff Satisfaction improved from 36% to 91% (+55%)
✨ Management Support increased from 24% to 89% (+65%)
✨ Reported burnout reduction improved from 36% to 86% (+50%)
✨ Workplace Culture climbed from 28% to 93% (+65%)

These numbers reflect more than satisfaction; they represent trust, belonging, purpose, and psychological safety. Most importantly, they have remained consistently high through 2024 and 2025, proving that this is not a one-time project, but a sustained cultural shift.
