The “Clinician Aid A” is the Ontario form for a patient to officially request a MAiD assessment. If signed and witnessed correctly it meets the legal criteria for a MAiD request. Once given to any Ontario Physician it must be acted upon with a MAiD assessment or with a timely and effective referral - this is required by the College of Physicians and Surgeons of Ontario, the body that licenses Ontario Physicians.
