Tooth 4.7 presented with infection associated with a crack. Periodontal probing revealed a 5 mm pocket on the distal aspect. During treatment, the crack was identified extending deeply into the distal canal. Although the exact depth of the crack could not be definitively determined, the presence of the 5 mm distal pocket suggested involvement at least to the cervical third.

Following completion of root canal treatment, the cervical third of the distal canal was restored with composite resin, similar to a conventional intracoronal restoration. The crown was then restored with composite in the same visit.
The prognosis for this type of case is considered guarded.
At follow-up, clinical examination demonstrated improvement, with the distal probing depth reduced from 5 mm to approximately 3–4 mm. Radiographic evaluation showed evidence of bone fill and healing in the cervical region corresponding to the crack.
Based on the clinical and radiographic findings, this case can be considered a successful outcome. Patient will have a full crown installed on the tooth.
