Covid-19 June 3, 2020 To All of Our Patients, We are working hard and doing our best to maintain patient care and assist our patient's with their conditions throughout this Covid -19 outbreak. If there is a change in this policy then we will send out further communications to all of our patients. Our office has resumed regular hours. Please read this communication carefully and if you have any questions please e-mail us at [email protected] For the safety of all our patients and staff we are performing enhanced screening for patients entering the office. This means the following: 1. All individuals entering the office must wash their hands with the provided hands sanitizer and fill out a screening questionnaire. Please wear a Mask. If you do not have a mask, we can supply one. Your forehead temperature will be taken with an Infrared non touch thermometer. You will be asked to remove any gloves that you may be wearing into the office. 2. If you have traveled anywhere outside of Canada including the United States or have close personal contact with anyone who has traveled outside of Canada including United States we ask you to not to come in for 2 weeks. If you are experiencing respiratory symptoms ( cough, fever, new onset of cough or shortness of breath ) or at least 2 of the following symptoms: chills, fatigue, headache, sore throat, runny nose, stuffy or congested nose, lost sense of taste or smell, hoarse voice, difficulty swallowing, digestive issues-(nausea/vomiting,diarrhea,stomach pain), or for young children and infants: sluggishness or lack of appetite, or if you have been in close personal contact with anyone who has been experiencing respiratory symptoms we ask you to please contact us at [email protected] and [email protected] (Please use both emails) or 905-881-4074 If you are currently being quarantined and are unable to attend your previously scheduled appointment please contact us at [email protected] or 905-881-4074 to reschedule your appointment.
