ANVH EP-71 IVDD in dogs 狗的椎間盤疾病

IVDD (Intervertebral Disc Disease) in dogs is a spinal condition that can cause pain, mobility issues, or even paralysis, especially in breeds with long backs and short legs. Early detection and treatment are crucial.
Here’s a comprehensive overview of IVDD in dogs:

What Is IVDD?
IVDD occurs when the intervertebral discs—the cushioning pads between the vertebrae—degenerate or rupture, putting pressure on the spinal cord and nerves. This can lead to symptoms ranging from mild discomfort to complete paralysis.

Breeds at Risk
Dogs with long backs and short legs are especially prone to IVDD, including:
• Dachshunds (25% may experience IVDD in their lifetime)
• Basset Hounds
• Corgis
• Shih Tzus
• French/English bulldog
These breeds often have a genetic predisposition that weakens spinal discs over time.

Types of IVDD
There are two main types:
• Hansen Type I: Sudden rupture of the disc, often seen in chondrodystrophic breeds (short-legged dogs). The disc material becomes calcified and can shoot out, causing acute pain or paralysis.
• Hansen Type II: Gradual degeneration, more common in larger breeds like German Shepherds. It causes chronic pain and spinal cord compression over months or years.
A third type may result from trauma or strenuous activity, causing spinal bruising without long-term compression.

Symptoms to Watch For

今天阿牛要介紹的是狗的椎間盤疾病
椎間盤疾病是指脊椎方面的問題造成疼痛,移動障礙,嚴重的case甚至癱瘓,特別是在一些特殊的犬種

椎間盤疾病通常是因為在脊椎骨中間的緩衝組織(椎間盤)發生腫脹,破損而壓迫到神經,進而引發的症狀,神經方面通常會引起較為劇烈的疼痛跟癱瘓,排便尿也會受到影響(神經控制)

好發犬種為:臘腸,柯基,西施,法國/英國鬥牛犬,巴吉度獵犬等
這些犬種因為基因上的缺陷,而特別容易出現這些問題,其中25
%的臘腸狗在一輩子狗生當中一定發生過IVDD

IVDD有分成三種類型:
1. Hansen type I:椎間盤突然的破損,常見於短腿體型的狗,椎間盤物質溢出且壓迫到神經,造成急性且劇烈的疼痛,有可能伴隨癱瘓或排便尿困難
2. Hansen type II: 椎間盤的退化腫脹造成神經的壓迫,較常發生於大型犬種,引起慢性疼痛,這問題通常伴隨著數月到數年的時間
3. 創傷造成,專間盤暫時性的水腫/瘀血造成壓迫,經過治療後恢復正常,通常不會留下後遺症

臨床症狀有:跛行,拱背,脖子僵硬,無法移動,劇烈的疼痛,癱瘓等
常見的檢查有理學檢查跟X光,想要確診部分必須做高階的影像學檢查

那我們怎麼治療?
口服消炎止痛藥,限制活動強迫休息,嚴重的case必須進行手術,接著是漫長的復健之路

這個疾病預防勝於治療,盡量避免垂直的運動,保持健康的體重,睡的床要有適當的支持,以及早期就醫

ANVH EP-70 Degloving trauma - tail amputation 脫套性傷害 - 截尾手術

A Niu received a phone call from an ambulance driver, said they found a dog with degloving trauma in the car accident scene...

Today A Niu would like to introduce a case of degloving trauma.

Degloving trauma in dogs is a severe injury where skin and underlying tissues are torn away, often due to being dragged or hit by a vehicle. It requires intensive wound care and sometimes multiple surgeries, but with proper treatment, recovery is possible.

Degloving injuries occur when a dog’s skin and subcutaneous tissue are forcibly separated from the underlying structures due to shear forces.
Common causes include motor vehicle accidents, especially when the dog is dragged or pinned.
The distal limbs (especially the medial tarsus/metatarsus) are most frequently affected.

🩺 Severity and Associated Damage:
Loss of skin, muscle, tendon, ligament, and bone
Neurovascular damage
Joint capsule pe*******on and instability
Embedded debris and bacteria, making the wound highly contaminated.

Treatment Approach:
Initial stabilization is critical—address life-threatening issues like blood loss before wound care begins.
Wound management
Multiple surgeries may be required depending on severity

上週,阿牛接到一通來自救護車司機打來的電話,說現場有一隻脫套性傷害的狗,問可不可以現在送來

今天阿牛要介紹的病例是"脫套性傷害"

脫套性傷害是指皮膚跟下面的軟組織嚴重的分離(脫手套一樣),常見於車禍,或其他機械性的創傷(剪力把軟組織捲入,造成嚴重的撕裂傷)
脫套性傷害通常位於肢端末段,而且傷口都很髒或有感染的風險

治療上通常第一步是先穩定病患,接著清理傷口,根據動物受傷程度可能需要多次的手術來修復手傷的組織,當遇到不可逆的傷害時則考慮手術截除

這個case是典型的尾巴脫套性傷害,不到一歲的狗在高速公路上突然跳車,造成了交通事故,幸運的是狗狗的生命現象穩定,經手術截除尾巴後恢復良好

ANVH EP-69 Abscess(膿瘍)

Today, A Niu would like to share a case of severe abscess in a golden retriever.

An abscess in a dog is essentially a painful pocket of pus caused by infection, trauma, or foreign objects. These can appear under the skin, in the mouth, or even internally. Common causes include bite wounds, splinters, or infected a**l glands. Symptoms often include swelling, redness, warmth, and sometimes a foul-smelling discharge. Dogs may also show signs of pain, fever, or behavioral changes like lethargy or loss of appetite.

Treatment usually involves draining the abscess and administering antibiotics. In some cases, surgical intervention may be needed.

This case is a 4-year-old golden retriever, came with an extremely swollen face. Initially, we think about the tumor or autoimmune issue. After examination, diagnosed with a huge abscess around the chin/R face area.

Our team performed a surgery to drain out more than 200ml pus, and put a draining tube into the wound. The dog improved a lot in 3 days.

今天阿牛要介紹一個嚴重膿瘍的病例

膿瘍是指在動物身上某個部位有一個囊袋狀的構造且裡面充滿膿汁,常見於創傷,感染or異物穿刺等等;他可以發生在動物的任何部位,在犬貓最常見的原因是打架
患部會有瘀青or紅腫,嚴重的病例會疼痛跟發燒(像這個case)

引流出膿汁是治療的第一要務,配合抗生素的治療,嚴重的病例需要手術介入(也是這個case)

在這隻4歲的黃金獵犬,整個右臉部跟下巴明顯腫脹,狗發燒到40度,不吃不喝;一開始還以為是腫瘤或者自體免疫的疾病,在經過檢查後,診斷為膿瘍

我們的團隊隨即手術介入,引流出超過200ml的膿汁,我們放了引流管進去傷口,3天後回診,腫脹的傷口已經好了很多,並移除引流管

ANVH EP-68 C-section in cat(貓的剖腹產)

A cat that walked into the hospital, and the owner said she had delivered a small kitten within an hour. Unfortunately, the first kitten died, and there's another one inside the mom...

A C-section in cats, or cesarean section, is a surgical procedure performed when a cat (queen) cannot deliver her kittens naturally due to complications like dystocia (difficult labor), fetal malposition, or uterine inertia. It’s often an emergency intervention to save both the mother and her kittens.

🩺 When Is a C-Section Necessary?
• Prolonged labor without delivering a kitten
• Gestation exceeding 65 days
• Fetal distress or death
• Maternal issues like a narrow birth ca**l or uterine rupture

After a quick ultrasound to confirm the fetus's status, a C-section was performed to help the mom deliver another kitten successfully.

Cesarean section is an urgent surgery. The earlier exam and diagnosis, the better prognosis for mom and the babies.
That's why timing is critical.

剖腹產是一個跟時間賽跑的手術，越早的診斷跟手術，母體跟胎兒的預後越好

ANVH EP-67 Scabies in dogs(狗的疥癬蟲感染)

Scabies in dogs—also known as sarcoptic mange—is a highly contagious skin condition caused by the Sarcoptes scabiei mite.

These microscopic mites burrow into the skin, triggering intense itching and inflammation.

Symptoms:
Severe itching and scratching
Hair loss, especially on elbows, ears, belly, and chest
Redness, crusting, and thickened skin
Restlessness or behavioral changes due to discomfort

Transmission:
Direct contact with infected dogs or wild animals (like foxes)
Contaminated bedding, grooming tools, or environments

Diagnosis:
Skin scrapings were examined under a microscope
Response to treatment may also help confirm the diagnosis

Treatment:
Prescription medications (oral or topical antiparasitics)
Medicated baths to soothe the skin
Isolation from other pets during recovery

Prevention:
Avoid contact with stray or infected animals
Regular vet checkups and parasite control

Scabies isn’t life-threatening, but it’s incredibly uncomfortable for your dogs and can spread quickly. If you suspect it, please contact your family vet ASAP.

今天阿牛要介紹的是一個很典型但是現在已經很罕見的皮膚寄生蟲-疥癬蟲(疥螨)
疥癬蟲會寄生在貓狗的皮膚底下，造成劇烈地搔抓，感染部位會增厚，發炎

常見的臨床症狀為劇烈的搔抓，病灶的位置會集中在肢體的末梢，包括手肘/下巴/吻部/耳朵/腹部等都是常見的地方，患部皮膚潮紅增厚帶有白色加皮脫落

最直接的傳染方式就是接觸傳染，跟其他的狗玩or野生動物打架，或者是汙染的動物床鋪，美容用具，環境等等。

疥癬蟲感染的狗會有典型的耳-足反射，就是搓耳朵會造成踢腳搔癢的行為，小時候路邊常見的的范皮膚流浪狗，大都是疥癬蟲的感染(如圖)

治療方式為口服或局部的驅蟲藥，加上藥浴，跟其他的感染動物隔離。

疥癬蟲在狗不是一個很緊急or致命的問題，但是感染根傳播的速度會很快，如果懷疑你的狗有類似的問題，請趕快跟你的家庭獸醫聯絡。

ANVH EP-66 Ear Hematoma in cats 貓的耳血腫

"Small Yellow" is a cat with heart failure hx with oral meds controlled, but its ear has grown 2 times bigger

Today A Niu would like to share a case of "Ear Hematoma"

An ear hematoma, also known as an aural hematoma, is a common condition in dogs and cats where blood collects between the layers of cartilage in the ear flap (pinna).

🩺 What Is an Ear Hematoma?
- A hematoma is a localized swelling filled with blood due to ruptured blood vessels.
- In the ear, this typically happens from vigorous head shaking or scratching, often triggered by:
- Ear infections
- Allergies
- Ear mites or fleas
- Trauma

Signs to Watch For
- Swollen, puffy ear flap that feels fluid-filled (like a water balloon)
- Head tilting or shaking
- Redness, warmth, or foul odor from the ear (suggestive of infection)
- Discomfort or pain when touched

How do we treat it?
- Mini surgery is the best solution
- Some cases can inject steroids into the ear pinna to control infection and inflammation

The specialty of this case is that the owner accidentally bandaged the ear pinna too tightly, and causing necrosis. A week later, the necrotic ear pinna fell off, the ear won't grow back, but it won't affect the life quality, except for the looking.

小黃是一隻有心臟病史的貓,經過藥物控制後心臟的狀況穩定,但是他的耳朵卻腫了起來

今天阿牛的分享的case是貓咪的耳血腫,之前講過了,再講一次,因為這個case很特別

耳血腫就是指貓或狗的殼中間血管破裂,血液充滿這個空間而腫起來,常見的原因為創傷或者劇烈的甩頭造成(貓狗的耳朵為軟骨,皮下的空間很大,且佈滿血管)

最常見的治療方式為局部手術,縫合皮下的死腔,等傷口癒合後便漸漸恢復,副作用則是耳朵通常會發生型變而皺縮;也有些case會施打類固醇到耳殼裡面來控制感染或發炎,不管使用哪種方式,術後的包紮是免不了的以避免持續地晃動

這個case特殊的地方在於主人意外的把耳朵包得太緊,導致耳殼的軟組織缺血壞死,一周後,壞死的組織自行脫落,動物的耳朵不會再長回來,所以形成一個一邊有耳朵,一邊沒耳朵的情況;幸運的是這並不會影響動物的生活品質,除了外觀

ANVH EP-65 Corneal ulcer in dogs and cats 犬貓的角膜潰瘍

Today, A Niu would like to share the cases of corneal ulcer in dogs and cats

🐶 What Is a Corneal Ulcer?
A corneal ulcer is an open sore or erosion on the cornea—the clear, dome-shaped surface covering the front of the eye. It can range from a superficial abrasion to a deep wound that threatens the eye's integrity.

Causes
Corneal ulcers can result from various factors, including:
- Trauma: Scratches from branches, cat claws, or rubbing the eye on rough surfaces
- Chemical burns: Exposure to irritants like shampoo or cleaning agents
- Dry eye (Keratoconjunctivitis Sicca): Leads to corneal drying and damage
- Eyelid abnormalities: Conditions like entropion (inward rolling eyelids) or distichiasis (abnormal eyelashes)
- Infections: Bacterial, viral, or fungal
- Breed predisposition: Brachycephalic breeds (e.g., Pugs, Bulldogs) are more prone due to their prominent eyes

Symptoms
Signs of a corneal ulcer include:
- Redness and swelling of the eye
- Excessive tearing or discharge
- Squinting or keeping the eye closed
- Pawing or rubbing at the eye
- Cloudiness or white film over the eye
- Sensitivity to light

Treatment Options
Prompt veterinary care is crucial. Treatment may include:
1. Topical antibiotics to prevent or treat bacterial infection
2. Pain management drugs to relieve discomfort and reduce inflammation

今天阿牛要分享的病例是貓狗的"角膜潰瘍"

角膜是是眼球表面透明的穹頂構造,負責調節光線的折射跟保護眼球,角膜潰瘍就是指這一層構造受傷(擦傷,穿刺傷等等)

在犬貓,最常見的原因為創傷(犬貓自己搔抓,打架,跟其他動物玩,自己磨蹭地板or牆壁),其他造成的原因像是乾眼症,系統性的細菌/病毒感染,品種特異性等等也都有可能會造成

臨床上的症狀為紅眼,眼睛無法正常張開,疼痛(進一步導致食慾下降or性格炯變),眼周分泌物多,動物會一直想要去抓眼睛or磨地板,角膜表面看起來混濁水腫,對光敏感等等

診斷上我們會用螢光染劑去染色,如果解模有受損,會根據受損的深度跟範圍呈現不同程度的螢光染色(一塊特別的亮,如圖)

輕微到中等程度角膜潰瘍會使用不同的眼藥水合併口服藥,嚴重的case可能需要進行手術來修補,但角膜潰瘍跟一般眼睛的感染不同的是需要更長的時間來康復

ANVH EP - 64 Crinal Cruciate Ligament rupture(CCL) 前十字韌帶斷裂

A 9-year-old overweight poodle, suddenly limping right after chasing a bird...

Today, A Niu would like to share a case of dog "CCL rupture".
A cranial cruciate ligament (CCL) rupture is one of the most common orthopedic injuries in dogs — similar to an ACL tear in humans.

🦴 What Is a CCL Rupture?
- The CCL stabilizes the knee joint by preventing the tibia from sliding forward under the femur.
- A rupture leads to instability, pain, and lameness, often seen as sudden limping or toe-touching with the affected leg.

Causes
- Degeneration over time: Most ruptures result from gradual weakening, not sudden trauma.
- Trauma or twisting injuries: Especially in active dogs making sharp turns.
- Breed predisposition: Larger breeds like Labradors, Rottweilers, and Golden Retrievers are more prone.
- Obesity: Increases stress on joints and risk of rupture.

Diagnosis
- Tibial compression test(Drawer test): Detects abnormal movement in the knee.
- X-rays: Reveal joint swelling, arthritis, or bone fragments.
- Sedation is needed for accurate evaluation, especially in tense or large dogs.

Treatment
- Either perform surgery to fix it, or try a conservative way

9歲的肥狗,追鳥追到一半突然跛行...

今天阿牛要分享的病例是狗的前十字韌帶(CCL)斷裂

前十字韌帶是一個位於膝蓋,負責穩定膝關節跟防止脛骨往前位移的構造,當這個斷裂的時候會造成膝關節的不穩定,立即性的跛行,伴隨著膝蓋腫脹跟疼痛

造成CCL斷裂的原因有很多,像是老化,劇烈的運動(跳躍/急轉彎),肥胖,跟特定的好發犬種(大型犬居多)

診斷上為拍攝X光還有進行抽屜試驗(必須在鎮靜狀況下,因為肌肉才會放鬆跟減緩疼痛)

治療上首選為外科治療(有許多不同術式,根據醫生的喜好以及動物的狀況決定,這個case是做TPLO),內科的保守治療通常只能緩解症狀,並不能恢復跛行的狀況

ANVH EP - 63 Testicle tumor(睪丸腫瘤)

A 9-year-old non-neutered Shiba Inu came to our hospital due to one testicle growth 4 times larger within 6 months.

Today, A Niu would like to share a case of a testicle tumor in a non-neutered male dog.

Testicular tumors are among the most common tumors in older, intact (non-neutered) male dogs, and they can range from benign to malignant.

Types of Testicular Tumors in Dogs
There are three primary types:
- Sertoli Cell Tumors
Develop from cells that support s***m development. These can produce estrogen and lead to feminization symptoms.
- Seminomas
Arise from germ cells (s***m-producing cells). Often benign but can be malignant in some cases.
- Interstitial (Leydig) Cell Tumors
Originate from testosterone-producing cells. Usually benign and asymptomatic.

Symptoms to Watch For
Signs can be subtle or absent, but common indicators include:
- Swollen or uneven testicles
- A firm or nodular mass in the sc***um
- Feminization signs (e.g., enlarged ni***es, hair loss, attraction to male dogs)
- Lethargy, weight loss, or decreased appetite (if cancer has spread)
- Difficulty urinating or defecating (due to enlarged lymph nodes or prostate)

The treatment methods include surgical removal, chemotherapy, and radiation therapy.

常見的臨床症狀有:
睪丸外觀腫大,不對稱/不規則,睪丸有硬團塊,排尿困難/疼痛,雌性化的表徵,厭食,體重下降等等

常見的治療方式有手術切除腫瘤,化療,放療,支持治療等等
這個case腫瘤經過切片,確診為良性的精細胞瘤,手術切除後即無大礙

ANVH EP - 62 Tibial crest fracture - 脛骨隆突(膝蓋生長板骨折)

A 6-month-old puppy suddenly screamed after jumping and fell backward. One leg is unable to bear the weight immediately, then send to the our hospital.

Today A Niu would like to share a case of "tibial crest fracture."

A tibial crest fracture in dogs—also known as a tibial tuberosity avulsion fracture—is a break at the front part of the tibia (shin bone) where the patellar ligament attaches. It’s most commonly seen in puppies and young dogs, especially large breeds, because their growth plates haven’t fully closed yet.

🦴 What Causes It?
1. Traction force from the quadriceps muscle pulling on the patellar ligament
2 .Jumping or sudden impact, especially during play or falls
3 .Underlying bone weakness in growing dogs

Symptoms to Watch For
1. Sudden lameness or inability to bear weight on a hind leg
2. Swelling around the knee, and pain when the area is touched
3. A noticeable bony bump or deformity near the stifle (knee)

Treatment Options
Surgical repair is typically required, especially if the fracture is displaced. This often involves: Pins to hold the bone in place,
sometimes a tension band wire for added stability

Recovery & Prognosis
1. Most dogs recover well with proper surgery and post-op care
2. Activity restriction for 6–8 weeks
3. Follow-up X-rays to monitor healing
4. Physical therapy may help restore strength and mobility

今天阿牛要介紹的病例為狗的脛骨隆突骨折(膝蓋生長板骨折)
脛骨隆凸就是膝蓋前端突起的位置,這構造在大部分的哺乳類動物都有,小時候會有個空隙,長大後會慢慢癒合(所以根據X光這個部位可以判斷是不是年輕的動物)

這個問題常見於大型犬,年輕有活力的狗,因為主要造成原因是巨大的反向拉力造成生長板無法承受而斷裂

臨床症狀為立即跛行(非常痛)及膝蓋周圍軟組織腫脹

治療方式為手術修復(保守治療通常沒什麼太大的幫助),大原則即把生長板固定回正常的位置,等他自己長好之後即癒合完畢,但手術後必須嚴格的限制活動至少6-8周的時間,而通常有這個問題的狗都非常好動,所以術後的照顧跟醫囑的遵守是一大挑戰
