Community Health Navigator (CHN)
Term: March 2026 – March 2027 with possibility of extension
FTE: .6 – 1.0 (negotiable)

The Rocky Mountain Primary Care Network (RPCN) is a physician-led non-profit corporation that works collaboratively with Primary Care Alberta and our community partners to support primary care in Rocky Mountain House and surrounding area.
We are looking for dynamic individuals who want to help patients that are not getting the medical help and social support they need. This position will work with Rocky PCN patients to improve access to health care and other resources available in their communities, connecting them with the broader healthcare system. This position is integrated within the Rocky PCN’s geographic community so they can promote the role and responsibilities as well as Rocky PCN programs and services. This is a non-regulated health care worker position, and supportive supervision and extensive on-the-job training will be provided. The successful candidate must be willing to participate in a formal training program led by a team of subject matter experts. This is a one-year term position, with the possibility of an extension. FTE is negotiable.

Qualifications and Experience requirements:

• Social Work and Nursing background an asset.
• Demonstrated community involvement that reflects strong ties to local community.
• Experience working with different groups and backgrounds.
• Training in Mental Health First Aid, Applied Su***de Intervention Skills Training (ASIST), Non-Violent Crisis Intervention (NVCI) an asset.
• Shared experience with targeted patient population an asset.
• Familiarity with medical terminology an asset.

Knowledge/Skills & Abilities:

• Strong desire to help and connect patients with the community.
• Strong community links within Rocky PCN catchment area.
• Willingness to learn health care terminology.
• Understanding of the healthcare and social resources available in area.
• Multicultural competence.
• Organizational and prioritization skills.
• Ability to teach others.
• Empathy and strong communication and interpersonal skills.
• Problem-solving skills.
• Ability to develop strong relationships with internal and external parties.
• Ability to discuss sensitive topics with patients and maintain confidentiality.
• Willingness to participate in a comprehensive formal training program.

This position reports to the Executive Director of the Rocky Mountain House Primary Care Network.

Salary: Will be negotiated based on experience. The PCN offers a competitive compensation package with comprehensive benefits, opportunities, and an outstanding work environment.

The Rocky Mountain Primary Care Network (RPCN) is a physician-led non-profit corporation that works collaboratively with Primary Care Alberta and our community partners to support primary care in Rocky Mountain House and surrounding area.

We are seeking an engaged and enthusiastic Medical Office Assistant/Receptionist to join our multi-disciplinary healthcare team. This position will support approximately twenty physicians by identifying and tracking panels, managing data and reporting information to the primary care practice population. This is a one-year term position, with the possibility of an extension. FTE is .6 – .8 and is negotiable.

Duties and Responsibilities:



· Assess and communicate the need for patient health screening within the EMR.

· Play a key role in coordinating screening referrals and appointments for patients in accordance with the Toward Optimized Practice Screening Initiative: Alberta Screening and Prevention Program. (ASaP).

· Work in conjunction with the Physicians and Panel Management Screening Nurse(s) to increase access for patients, enhancing the patients’ primary care experience, improving clinical outcomes and the patient’s quality of life.

· The Receptionist/MOA works under general supervision, frequently in the absence of managers and senior staff.

· Support the ED and office administrative functions.

· Reception: answering phones, mail distribution, minutes, document management, data entry and database maintenance.



Requirements:



· Medical Office Assistant, Unit Clerk, Medical Terminology certification preferred or combination of education and relevant experience.

· Experience in with multiple-line phone systems and reception.

· Demonstrated ability and willingness to work in an unstructured and dynamic environment that requires flexibility and collaboration with other health professionals.

· Must be prepared to increase knowledge in current screening initiatives and research within the Alberta and Canadian Context.



Knowledge/Skills & Abilities:



· Sound knowledge and experience with computer programs in Microsoft Office.

· Previous experience with Electronic Medical Record (EMR) and working in clinical settings would be an asset.

· Ability to collaborate with people in a health care environment with professionalism, confidentiality, and respect.

· Outstanding communication skills, verbally and in writing.

· Proven initiative and motivation.

· Strong organizational, time management and decision-making skills; meeting deadlines.

· Ability to deal with challenging situations with empathy & sincerity – conflict resolution.

· Demonstrated problem solving and conflict resolution skills.

· Highest standards of confidentiality, privacy and information management.

· Ability to work effectively within a team.

Hello everyone! Just putting out a reminder that the Rocky Primary Care Network has moved locations!

We are in our brand new space now, located at Unit 1 - 5020 44 Street (beside Sobeys).

We sincerely apologize that we currently do not have new signage up yet, but are working towards getting that done soon.

We are in the section of the building shown in the photo (our door is closest to the baseball diamonds).

Please note that the Rocky Medical Clinic has NOT moved locations yet, however, we look forward to them joining us in the new space in the future.
