I regret to inform you that I, Dr. Umarah Farrukh, will be closing my medical practice, and will no longer be available to provide your medical care effective July 31st, 2024. I will be available until that time for your health care needs. Please use this time to book an appointment in the clinic to ensure prescriptions are up to date and/or screening, required medical follow up and lab work has been obtained prior to my departure.

Please select another physician within this time frame to continue your medical care. There will be a new family physician joining our community starting around the middle of May 2024. You can also search for a new family physician at Find A Doctor (albertatindadoctor. ca), this resource will provide assistance in finding another physician.

You are welcome to request a copy of your medical records be sent to a new physician once one has been found or alternatively a copy can be provided to you upon your request. If you are unable to find a new physician within this designated timeframe, records will remain available upon request even after my departure.

Please contact The St. Paul Medical Clinic at 780-614-3210 to book your appointment as required with myself. The St. Paul Medical Clinic will remain open with other physicians and staff after my departure.
